Topical Steroids under Wet-Wrap Dressings in Atopic Dermatitis – A Vehicle-Controlled Trial

Abstract: Background: The wet-wrap dressing technique has proved to be beneficial in cases of exacerbated atopic dermatitis (AD) skin lesions. Objective: The effect of wet-wrap dressings was investigated in a controlled trial comparing a steroid (mometasone furoate 0.1%)-containing and a steroid-free (vehicle) preparation in an in-patient comparison study. Methods: 20 children aged 2–17 years with exacerbated AD were treated twice daily with wet-wrap dressings over a 5-day period. Results: AD in treated areas significan… Show more

“…At the same time the dressings prevent scratching, due to their textile barrier effect on the inflamed skin [11]. Patients with AD feel comfortable with wet-wraps on their inflamed skin due to these effects [14], whereas this treatment does not lead to a bacterial superinfection [7]. …”

“…The application of wet-wrap dressings in patients with AD has shown to be effective using emollients only [4], emollients in combination with antiseptics [5], or in combination with topical corticosteroids [6]. Schnopp et al [7] compared the use of the wet-wrap therapy with a topical corticosteroid preparation versus a corticosteroid-free basic ointment. The study confirmed the efficacy of both wet-wrap dressing techniques for the treatment of exacerbated AD lesions.…”

“…On healthy skin in adults, merely 0.4% of the topically applied mometasone is systemically absorbed [22], which is likely too little to produce a therapeutic effect. Furthermore, in another study that compared wet-wrap dressings with and without topical steroids, the result was much more beneficial for the steroid-treated body parts [23]. Finally, a certain placebo effect may have contributed to the general improvement of eczema in all areas of the body during the first week, and this placebo effect may have diminished towards the end of the study.…”
