Topical capsaicin for chronic neuropathic pain in adults

Abstract: Background-Topical creams with capsaicin are used to treat pain from a wide range of chronic conditions including neuropathic pain. Following application to the skin capsaicin causes enhanced sensitivity to noxious stimuli, followed by a period with reduced sensitivity and, after repeated applications, persistent desensitisation. There is uncertainty about the efficacy and tolerability of capsaicin for treating painful chronic neuropathies.

“…However, even in this arena, evidence for the effectiveness or safety of capsaicin use in pain relief is controversial. Application of capsaicin to the skin causes an enhanced sensitivity to noxious stimuli, followed by a period with reduced sensitivity and, after repeated applications, persistent desensitization (37). Capsaicin-induced dermal pain is common after exposure to capsaicin-containing hot peppers, personal protection sprays, or topical creams (38).…”

“…Capsaisin is already commercially available as dermal formulation in pain relief and local inflammatory diseases (e.g. Stilene s , RadoSalil s ) and is being investigated in several clinical trials for several indications which have been reviewed manifold (Bode and Dong, 2011;Derry et al, 2009;Qureshi et al, 2008;ReyesEscogido et al, 2011;Wallace and Pappagallo, 2011). Various Echinacea formulations (e.g.…”
