Initial attachment of human oral keratinocytes (HOKs) cultured on mirror-surfaced commercially pure titanium (Ti) or yttriastabilized tetragonal zirconia polycrystals (TZP) was investigated. Numbers of viable attached HOKs, their mRNAs and proteins expression of laminin γ2 and integrin β4 were evaluated using the WST-1 assay, quantitative real-time PCR and enzyme-linked immunosorbent assay, respectively. Localization of laminin γ2 and integrin β4 was observed using immunofluorescent staining. Cell spreading was evaluated by measuring the perimeter of actin on fluorescent stained images, and cell morphology was examined using scanning electron microscopy. At 1 h TZP elicited less of initial attachment than Ti in terms of mRNAs expression and proteins expression of laminin γ2 and integrin β4 (p<0.05). However, at 48 h TZP was showed similar initial attachment in comparison to Ti. Therefore, it was suggested that TZP has a potential to form epithelial attachment like Ti.

Résumé -En chirurgie bucco-dentaire, le praticien, le personnel et les patients sont de plus en plus confrontés à des phénomènes d'allergie et d'intolérance. Ces phénomènes sont fréquents et leur prévalence a tendance à s'accroitre principalement à cause de la diversité et de la multiplication des produits allergisants qui sont manipulés. Les allergènes responsables ne sont pas toujours bien documentés. Parmi ces allergènes, le latex est le plus fréquemment cité dans la littérature et les réactions allergiques ont une prévalence variant entre 2,8 % à 16,9 % chez les professionnels de santé. Il serait responsable d'allergies cutanées et respiratoires de plus en plus fréquentes. Il est ainsi devenu une source de préoccupations pour les allergologues d'où l'intérêt de bien définir les allergènes responsables, les facteurs déclenchants, le pouvoir pathogène ainsi que les différentes expressions cliniques de cette allergie. Les matériaux prothétiques manipulés par le chirurgien dentiste et le prothésiste, notamment les résines acryliques et certains matériaux d'empreinte ainsi que les métaux (nickel, cobalt, chrome, or) ont été également incriminés dans la survenue des manifestations allergiques. Le titane, métal considéré comme ayant une excellente biocompatibilité, a également fait l'objet de nombreuses publications, ces dernières années, montrant son rôle dans la survenue de réactions allergiques. Rarement, les antiseptiques utilisés en pré-opératoire ainsi que certains désinfectants peuvent être responsables d'intolérance ou de sensibilisation de contact. Enfin, les anesthésiques locaux sont incriminés, souvent de façon abusive, dans des manifestations considérées comme des réactions allergiques. Les réactions allergiques peuvent être banales à type d'érythème muqueux chez les patients ou à type d'urticaire, voire d'eczéma cutané chez les praticiens et le personnel, mais elles peuvent prendre d'autres formes : oedéme de Quincke pour le latex, échec implantaire inexpliqué. La difficulté principale réside dans la recherche de l'étiologie de ces accidents allergiques. L'interrogatoire est d'une grande importance pour poser le diagnostic, pour dépister les patients à risque mais également pour mettre en oeuvre les moyens de prévention ; toutes ces démarches doivent être effectuées en étroite collaboration avec l'allergologue.Abstract -Allergens in dentistery. Because of their outstanding mechanical properties, chemical stability, and biocompatibility, 3-mol % yttria-stabilised tetragonal zirconia polycrystals (3Y-TZP), known as zirconia ceramics in dentistry, are an important choice for various types of prosthesis. In addition to extensive use for crown and bridge construction, considerable interest has been generated for applications in implant dentistry, including full-contour zirconia crowns as supra-constructions, zirconia abutments, and novel zirconia implants. However, their use among dentist and researchers is controversial, especially compared with the well-established implants made of titanium alloys. As a latecomer, the merits and limitations of 3Y-TZP are awaiting careful investigation. Design, manufacturing, and clinical operation guidelines are urgently needed. The aim of this review was to address the present status of the application of zirconia ceramics related to implant dentistry by analysing the published data from both in vitro and in vivo studies. Suggestions are provided for potential improvements and suitable applications of zirconia ceramics in metal-free implant dentistry. ARTICLE HISTORY

Abstract: Ti allergy can be detected in dental implant patients, even though its estimated prevalence is low (0.6%). A significantly higher risk of positive allergic reaction was found in patients showing post-op allergy compatible response (ACRG), in which cases allergy tests could be recommended.
