Tissue Velocity Echocardiography Shows Early Improvement in Diastolic Function With Irbesartan and Atenolol Therapy in Patients With Hypertensive Left Ventricular HypertrophyResults From the Swedish Irbesartan Left Ventricular Hypertrophy Investigation vs Atenolol (SILVHIA)

Abstract: The TVE is more sensitive than conventional echocardiography in detecting alterations in diastolic function. Both irbesartan and atenolol improve diastolic function, but through different mechanisms. The improvement in IVRTm was greater with irbesartan, and only irbesartan improved E/Em. This may have implications on the treatment of high risk hypertensive patients.

“…This difference was also accounted for by differences in heart rate. A previous study also observed an improvement in the E/A ratio after 48 weeks of treatment with atenolol, which was highly correlated with the reduction in heart rate (8). We, therefore, suggest that E/A ratio is not a particularly useful indicator of diastolic function when heart rate differs between treatments.…”

“…While many studies have focused on the effectiveness of hypertension treatment in reducing cardiac hypertrophy, less is known about the impact of treatment on LV diastolic function (8,9). Previous studies addressing the impact of different antihypertensive agents on LV diastolic function have largely used conventional echocardiography assessing transmitral filling and isovolumic relaxation.…”

“…However, it is ethically difficult to conduct cardiac catheterization only to assess LV filling pressure and diastolic function. In addition, the E/e′ ratio is recognized as an established index to assess LV filling pressure and the severity of diastolic dysfunction in the chronic stable stage (24) and is now widely used even in clinical trials (31)(32)(33).…”

“…14 LVH and LV diastolic dysfunction have been widely recognized as predictors of cardiovascular morbidity and mortality in hypertensive patients. 15,16 Subtle modifications in LV structure and geometry may occur in the early phase of EH, 17 and myocardial fibrosis plays a major role. 14,17 Accordingly, many investigators are interested in the quantification of myocardial fibrosis.…”
