Tip rhinoplastic operations using a transverse columellar incision

Abstract: In more than 300 cases of aesthetic and corrective rhinoplasty, the author's experience should encourage plastic surgeons to consider the use of the transverse columellar incision for better exposure of the nasal tip area. Five case studies are briefly included and the author encourages the reader to look upon this operative approach with more favor and not consider it a technique to be tabooed or avoided.

“…The m. compressor narium minor runs from the area of the genu of the alar cartilage to the integument of the very tip of the nose. It is likely that there is considerable variation in the nasal muscular anatomy of this area, but in many individuals, clinical and peroperative evidence of muscular presence near the tip of the nose is found [12,13,15].…”

“…We claim that the best analyzed scar shape is represented by the Vscar, with the incisions properly positioned, i.e. far from the base of the nose, in accordance with medical evidence (Gunter, 1997;Nolst, 2005;Toriumi et al, 1999;Celik et al, 2003;Voight, 1983). The surgical issues of wound orientation with respect to tension lines, wound shape and geometry so as to minimize scarring has been usually addressed from a mechanochemical point of view and at the cellular scale (Dale et al, 1995;Olsen et al, 1995).…”

“…Although the shape of the incision may vary, geometric-type incisions, with multiple corners, are preferred to prevent long scar segments and facilitate flap alignment. Final scars with these types of incisions are qualitatively superior to horizontal or transverse I scars (Voight, 1983).…”
