Three-dimensional analysis of glomerular morphology in patients with subtotal nephrectomy

Abstract: Previous studies documented that single section examination of kidney tissue underestimates glomerulosclerosis and that three-dimensional examination of glomerular morphology improves recognition of the incidence and distribution of sclerotic changes within the glomerular capillary tuft. We have adopted this technique to evaluate the true frequency and the spatial extent of glomerulosclerosis in patients who were subjected to extensive renal mass reduction. We re-evaluated four kidney biopsies of patients with… Show more

“…In all of the studies described above, quantification of sclerosis was based on semiquantitative scores. Yet 2D analysis of a single section of a biopsy specimen overestimates the number of normal glomeruli and underestimates the actual extent of glomerulosclerosis (106,107). Thus one cannot currently tell what volume of glomerular capillaries is actually affected by sclerosis and to what extent the sclerosis volume is effectively reduced by treatment.…”

“…The complex three-dimensional structure of the glomerular capillary bed has been studied using serial section microtomy (16,50,76,90). However, the method generally has not been practical for studying the overall renal vascular architecture because of physical problems relating to the alignment and registration of adjacent sections, lost or distorted sections, and time and effort involved in the preparation and analysis of the sections.…”

“…Thus, while only an average of 29% of glomeruli were scored as affected by glomerulosclerosis in the adriamycin model on single sections, serial-section reconstructions revealed that 92% of glomeruli on average actually showed sclerotic changes. Similar results have been obtained in the analysis of human kidney biopsies (14). To accurately establish to what extent AII antagonism can effectively reduce the sclerotic areas, and the volume of glomerular capillary tuft eventually regenerated, more extensive investigation, focused at the individual glomerular level, is needed.…”
