Therapeutic Efficiency of Platelet Gel for the Treatment of Oral Ulcers Related to Chronic Graft Versus Host Disease After Allogeneic Haematopoietic Stem Cell Transplantation

Abstract: These data confirm that the PLT gel may be used as a safe and effective tool, alone or in combination with systemic therapy, for the treatment of mucosal lesions of mouth related to cGvHD.

“…Researchers used autologous and allogeneic PG to treat oral cGvHD, the data confirmed that the PG might be a safe and effective tool, alone or in combination with systemic therapy, for the treatment of mucosal lesions of mouth related to cGvHD. 8,71 and 20 patients were treated with conventional treatment (compression and dressing) for 6 weeks. The mean percentage improvement in the area of the ulcer post-PG and conventional therapy was 67.6% ± 36.6 and 13.67% ± 28.06%, respectively.…”

“…Platelets for PG come from patients' own blood, 5,8,54,55,57,60,65,68,71,76 donors' blood, 56,58,[61][62][63]67,69,72 and cord blood. 59,[73][74][75] Advantages of APG are no need for blood type matching, without risk of transfusionrelated infections, and easy to collect blood.…”

“…One study showed that after two PG applications, all 10 patients had relief in oral pain with better oral intake, and after a median of five applications (range, 2–15), 70% showed a CR. No significant AEs occurred except for mild, self‐limited oral itching or burning sensation 108 . A second study reported on five patients who received a median of six (range, 2–8) PG applications, with overall response of 43·2% and 27·3% per the oral mucositis rating scale and NIH cGVHD oral mucosal score respectively 107 …”

“…Two case series were published in the past five years on the safety and efficacy of autologous platelet gel (PG) topical application for oral cGVHD 107,108 . Platelet derivatives are rich in cytokines and growth factors, shown to play a role in tissue repair and healing, 109 making this an attractive therapeutic approach in cGVHD.…”

“…Oral ulcers can also occur after bone marrow allogeneic transplants resulting in graft vs. host disease (GvHD). Application of PCs gel in those patients reduces pain, enhances mastication, and accelerates healing (107). In addition, PRF membrane could improve tissue healing after excision of oral mucosal lesions such as leukoplakia and lichen planus (108).…”
