The Wrestler's Ear (Acute Auricular Hematoma)

Abstract: \s=b\The wrestler's ear (acute auricular hematoma) is a common problem for physicians treating facial trauma. One method of treatment involves open drainage of the serum and excision of the newly formed fibroneocartilage. This technique provides an outstanding cosmetic result and allows the athlete continued wrestling competition. (Arch Otolaryngol 1985;111:161-164)

“…In treating these lesions, careful dissection of this tissue prior to the application of an appropriate pressure dressing must be carried out to prevent the deformity of cauliflower ear. Various types of pressure dressings have proved effective in preventing the reaccumulation of the hematoma, including compression sutures, 6,7 several types of cotton bolsters, 5,8 silicone rubber splints, 9 and a removable auricular stent. 3 …”

“…Unfortunately, this technique was not efficient, which was the reason why varied other techniques began to appear in medical journals. Some of these included the window operation (24), aspiration and plaster molding of the auricle (25), ultraviolet exposition (26), continuous portable vacuum drainage (27), button technique (28), and drainage and excision of the fibroneocartilage (29). The management of auricular hematoma has always been a challenge for physicians.…”

“…8,9,11 Treatment goals for the management of auricular hematoma include adequate removal of the hematoma, a simple and effective method of maintaining pressure to prevent reaccumulation of fluid, satisfactory aesthetics, appropriate follow-up, and minimal impact on patient activity. 1,7,13,14 In the case reported herein, several things were considered to achieve these goals. Based on the patient's underlying blood dyscrasia, it was considered imperative to maintain adequate pressure over a broad area to prevent reaccumulation of the hematoma.…”

“…An incision was made to gain access to the hematoma because aspiration alone is seldom complete and is often associated with a reaccumulation of blood or seroma, even in the presence of a pressure dressing. 1,8,9,11,14,15 Also, in light of the patient's easy bruisibility and thrombocytopenia, it was thought that the potential for repeated aspirations represented a contraindication to this technique. The splint used in this case also fulfilled the treatment objectives of cosmesis and function.…”
