The White-Eyed Medial Blowout Fracture

Abstract: Orbital floor blowout fractures in the pediatric population have a high incidence of muscle entrapment that must be recognized and treated early to avoid muscle necrosis and permanent ocular restriction from fibrosis. Medial orbital wall fractures with entrapment are rare, but early recognition and operative release of the entrapped muscles result in better outcomes.

“…10,11 This report pointed out that accidental injuries were the nation's most important environmental health problem. This was followed by another important publication "Injury in America" which documented in much greater detail the impact of injuries on American society and suggested a broad approach to the problem encompassing epidemiology, prevention, biomechanics, acute treatment, and rehabilitation.…”

“…Although the medial orbital wall is thinner than the floor, the floor is more susceptible to fracture because the floor has no central support. Conversely, the medial wall has many bony septa within the ethmoid sinus that support the wall and make deformation less likely [4, 5]. Thus, orbital fractures of the medial wall are reported, uncommonly.…”

“…Some reported that most blowout fractures involved the orbital floor with only 4–12% involving the medial wall [1, 4]. The proposed mechanism of blowout fracture includes the hydraulic and buckling theories [4]. The trapdoor-type orbital fracture can be explained by buckling theory.…”
