The Value of Direct Immunofluorescence on Proteinase-Digested Formalin-Fixed Paraffin-Embedded Skin Biopsies

Abstract: Direct immunofluorescence (DIF) on frozen tissue (DIF-F) is the method of choice for the identification of immune deposits present in skin and other tissues. DIF can also be performed on formalin-fixed paraffin-embedded tissue (DIF-P) after antigen retrieval with proteases and has proven to be of value in renal pathology. However, its utility in skin biopsies has not been fully examined. In this study, we performed DIF-P on 60 skin biopsies that comprised of bullous pemphigoid (n = 18), pemphigoid gestationis … Show more

“…We hypothesized that the preceding LP‐like lesion induced by PD‐1 inhibitor evoked the production of autoantibodies to epidermal BMZ. To confirm this, we performed a DIF assay of formalin‐fixed, paraffin‐embedded (FFPE) specimens biopsied from the LP‐like lesion after antigen retrieval, since DIF assays of FFPE specimens are useful for the detection of autoantibodies to epidermal BMZ 4 . DIF assay revealed IgG deposition at the epidermal BMZ (Fig.…”

“…For DIF, fresh frozen tissue is preferred compared with formalin-fixed paraffin-embedded (FFPE) tissue, but DIF on trypsin digestion of formalin-fixed sections can be an alternative method when fresh frozen tissue is unavailable. [4][5][6] In addition, if only FFPE tissues are available, C3d or C4d immunohistochemistry (IHC) has also been considered as a valuable diagnostic tool for bullous pemphigoid (BP) and MMP. [7][8][9][10][11] IHC for C4d or IgG4 may be diagnostically useful in pemphigus, 10,12 and C4d IHC can be helpful in the diagnosis of pemphigoid gestationis.…”
