The Utility of the In Situ Detection of T-Cell Receptor Beta Rearrangements in Cutaneous T-Cell–Dominant Infiltrates

Abstract: The diagnostic assessment of cutaneous T-cell infiltrates is problematic for dermatopathologists. A variety of conditions, including lymphomatoid hypersensitivity reactions and lymphomatoid lupus erythematosus, can demonstrate lymphoid atypia and phenotypic changes that can mimic cutaneous T-cell lymphoma (CTCL). A similar issue revolves around lymphoid dyscrasias, which includes parapsoriasis, atypical pigmentary purpura, pityriasis lichenoides chronica, indeterminate lymphocytic lobular panniculitis, and lym… Show more

“…5 However, they must be interpreted with caution because clonality can be present in other diseases such as cutaneous T-cell lymphomas, parapsoriasis, lymphomatoid lupus profundus, atopic dermatitis, pityriasis lichenoides chronica, pigmentary purpura, and drug-associated lymphomatoid hypersensitivity. [15][16][17] Here, we describe the clinical, histopathologic, and molecular features of LyP in pediatric patients (aged \ 20 years).…”

“…3,21 Clonal TCR rearrangements have been noted in benign skin disorders, and the results must be interpreted with caution. 21,22 Little is known about the mechanisms involved in skinspecific homing of cells in LyP. 12,23,24 Previous studies have shown that CD4 + and CD30 + -positive LyP cases were TARC, CCR4, and Mig negative.…”
