Treatment of superficial fungal infections has come a long way. This has, in part, been through the development and evaluation of new drugs. However, utilising new strategies, such as identifying variation between different species in responsiveness, e.g., in tinea capitis, as well as seeking better ways of ensuring adequate concentrations of drug in the skin or nail, and combining different treatment methods, have played equally important roles in ensuring steady improvements in the results of treatment. Yet there are still areas where we look for improvement, such as better remission and cure rates in fungal nail disease, and the development of effective community treatment programmes to address endemic scalp ringworm.

BACKGROUNDOnychomycosis is a fungal infection of the nails caused in most cases by dermatophytes Trichophyton rubrum and Trichophyton mentagrophytes. Despite numerous available antifungal drugs for therapy of this infection, the cure rate is low, with high rates of relapse after treatment and side effects.OBJECTIVESTo present a new option for the treatment of onychomycosis, in search of a more effective and rapid method than conventional ones.METHODSPatients underwent two sessions of CO2 fractional laser 10.600nm associated with photodynamic therapy. Mycological and digital photography were performed before and after the treatment.RESULTSMcNemar test with continuity correction and degrees of freedom = 1: for clinical cure rate, 13.06, with p=0.00005; for mycological cure, 17.05, with p=0.00005; 72% felt fully satisfied with the procedure.CONCLUSIONSThe use of fractional CO2 laser 10.600nm associated with photodynamic therapy can be effective in the treatment of onychomycosis, decreasing the risk of systemic lesions that may be triggered with prolonged use of oral antifungals.

Przegląd Dermatologiczny 2016/3 246 STRESZCZENIE Grzybica paznokci jest częstą chorobą dotyczącą aparatu paznokciowego, a jej leczenie wg obecnie obowiązujących standardów nie zawsze przynosi zadowalające efekty. Celem pracy jest zwrócenie uwagi na nowe możliwości terapii z wykorzystaniem promieniowania laserowego, promieniowania ultrafioletowego typu C, a także terapii fotodynamicznej. Najwięcej danych piśmiennictwa dotyczy wykorzystania lasera neodymowo-yagowego, a także laserów diodowych lub ablacyjnego lasera dwutlenkowo-węglowego. Niektórzy autorzy, aby poprawić skuteczność leczenia, stosowali przed zabiegiem usuwanie mas grzybiczych lub dołączali miejscowe leczenie preparatami przeciwgrzybiczymi. Ponadto przedstawiono metodę fotodynamiczną wykorzystywaną w leczeniu stanów przednowotworowych i powierzchownych nowotworów skóry, która daje obiecujące wyniki w terapii grzybicy paznokci z zastosowaniem tych samych lub innych fotouczulaczy. Uzyskane przez autorów efekty leczenia i brak poważnych działań ubocznych zachęcają do stosowania tego typu terapii, ale żadna z metod do tej pory nie została porównana z leczeniem farmakologicznym wg aktualnych wytycznych. ABSTRACTOnychomycosis is a frequent nail disease. Standard treatment is not sufficient in some patients. The aim of the study is to provide an overview of laser, UVC and photodynamic therapy of onychomycosis. The use of Nd:YAG laser is described in many articles, and also diode lasers and ablative CO 2 are used. Some authors, in order to improve the efficiency of treatment, also administered topical antifungal therapy or removed a part of the nail. Also photodynamic therapy usually used in precancerous diseases or superficial skin tumors is also employed in onychomycosis with the same or new photosensitizers. The results and the lack of serious side effects encourage the use of these methods, although none has been compared to standard oral therapy.
