The Use of High-Resolution Ultrasonography for Preoperative Detection of Metastases in Sentinel Lymph Nodes of Patients with Cutaneous Melanoma

Abstract: High resolution B-mode US cannot replace SLNB, especially in the detection of micrometastases, but it remains the most important method to assess the lymph node status for macrometastases presurgically.

“…[12][13][14]38,41,42 Several published reports have described the sensitivity, specificity, and accuracy of US for LN metastases, primarily for breast applications. [12][13][14][15]38,41,42 In patients with breast cancer, the lymphatic drainage pathways are relatively predictable, whereas in patients who have melanoma, the location of the SLNs is more variable. 43,44 This project has confirmed the potential utility of lymphosonography as a lymphatic mapping technique and its ability to localize SLNs even in cases of unpredictable drainage pathways.…”

“…[12][13][14] It has also been found to be a valuable method to guide LN biopsies. 15,16 Although gray scale US, color flow US, and pulsed Doppler imaging have been used alone or in combination to assess LNs for the presence of metastases, US cannot be used for lymphatic mapping (ie, to identify a tumor's SLNs) because mapping requires administration of a tracer (eg, dye or radiopharmaceutical).…”

“…These authors hypothesized that the cap corresponded to the region where the melanoma-specific afferent lymphatic vessel enters the lymph node. In our opinion, as also suggested by other authors who could not see the cap phenomenon, 25,31,32 these findings are too subtle to be used in clinical practice, where slight asymmetry of the nodal cortex is not an uncommon finding.…”

“…We take the shape of the lymph node into consideration, being suspicious when oval and even more suspicious when round and not being suspicious if markedly elongated. Some authors consider an L/T ratio of less than 2 suspicious, 8,19,24,25 whereas others, as well as us, con-sider an L/T ratio of less than 1.5 suspicious. 7,18,26 We believe that a threshold of 1.5 increases the rather low specificity of the size criterion.…”

“…In a recent smaller study including 25 consecutive patients with cutaneous melanoma, Kunte et al [44] evaluated the ability of high-resolution B-mode sonography (without color Doppler sonography) for preoperative identification of SLNs. In 25 patients, 35 SLNs were removed surgically and 6 (17.1%) turned out to be positive by histopathologic examination.…”
