The Use of Cognitive Testing to Develop and Evaluate CAHPS™ 1.0 Core Survey Items

Abstract: Cognitive testing was integral in the development and refinement of the CAHPS instrument. The cognitive testing findings contributed to an improved instrument that should capture consumers' health care and plan experiences with less response error than one not subjected to such testing. The cognitive testing process and findings can be useful to other researchers with similar survey development goals.

“…The measure was interviewer-administered as if in a routine interview and at the same time 'direct probing' techniques were used, involving the asking of supplementary questions that seek to examine how respondents interpret or feel about items and response options [12]. “…The cognitive interview method has played an important role in the development of many diverse health care measures. Examples include the Consumer Assessment of Healthcare Providers and Systems (CAHPS) surveys that target Medicare recipients' understanding of their health plan [6,7] and patient experiences with hospital care [6,8]; the digestive health status instrument to identify patients in community, primary care, and gastroenterology practices suffering from gastroesophageal disease [9,10]; and the Chicago Community Asthma Survey to measure public understanding of asthma and its management [11].…”
