“…These are effective for providing local anaesthesia to the lateral thoracic walls for several hours, with improved postoperative ventilation without the need of using high doses of systemic opioids. The local anaesthetic most commonly used is bupivacaine (0.5 ml bupivacaine 0.5% per site in patients up to 10 kg, and 0.5–2 ml bupivacaine 0.5% per site in patients greater than 10 kg), which is injected dorsally, just below the epaxial muscle insertion, at the caudal border of the ribs (Read and Schroeder, ). Bupivacaine can alternatively be administered intrapleurally along the thoracostomy tube (the total dose of 2mg/kg every 4 hours can also be divided between intrapleural and intercostal blockade).…”
