Objective: to evaluate the contribution of an instructional module identifying the number and characteristics of lymph nodes by undergraduate nursing students. Method: an experimental, randomized, controlled and masked study using an instructional module for intervention. The 68 students who made up the control group or the experimental group performed the prototype lymph node palpation in the initial and final phases, following the free registration of the characteristics and number of these structures. Between the phases, the instructional module (palpation prototype and a registration guide instrument) was applied to the experimental group. Descriptive statistics and mixed linear regression were used for analysis. Results: the experimental group showed greater accuracy (p<0.05) in the evaluation of the size, consistency, mobility and coalescence of the lymph nodes in the final phase when compared to the control; it also showed more chances to correctly evaluate the consistency ( OR 45,26; 95% CI<7,74>‡<264.54> p<0.0001), mobility (OR 55.95; CI 95% 12.45 - 251.60; p<0.0001) and size (OR 25.64; CI 95% 3.92 - 160.2; p=0.0002) of the lymph nodes. Conclusion: the results reinforce the contribution of the instructional module to increase the knowledge of nursing students about the evaluation of lymph nodes.

Influence of Personalized Health Management Model Based on Internet Mode on Self-Management Ability and Life Quality of Patients with Chronic Diseases Undergoing Physical Examination

Objective. To explore the influence of personalized health management model based on the Internet mode on self-management ability and life quality of patients with chronic diseases of physical examination. Methods. Sixty patients with chronic diseases who went through physical examination from February 2019 to April 2021 were enrolled in our hospital. The patients were arbitrarily assigned into control and research group. The former group received routine nursing, and the latter group received personalized health management mode based on the Internet mode. The nursing satisfaction, self-management behavior ability, blood pressure level, physiological index, mastery of health knowledge, and life quality scores were compared. Results. In terms of nursing satisfaction, the research group was very satisfied in 20 cases, satisfactory in 9 cases, and general in 1 case, with a satisfaction rate of 100.00%; In the control group, 15 cases were very satisfied, 9 cases were satisfied, 1 case was general, 5 cases were not satisfied, and the satisfaction rate was 83.33%; The nursing satisfaction of the research group was higher ( P < 0.05 ). Compared with the self-management behavior ability, the scores of diet control, regular medication, correct medication, and smoking cessation in the research group were remarkably higher ( P < 0.05 ). There exhibited no significant difference in blood pressure level before intervention ( P > 0.05 ); after intervention, the blood pressure levels lessened. Compared with the control group, the systolic blood pressure and diastolic blood pressure of the research group were lower ( P < 0.05 ). The levels of triglyceride, total cholesterol, low-density lipoprotein, and fasting blood glucose in the research group were remarkably lower ( P < 0.05 ). Compared with the control group, the scores of health knowledge in the research group were remarkably higher, including the harm of smoking, the use of drugs, the importance of persisting in taking drugs, the pathogenesis of chronic diseases, the effect of emotional management on the disease, and the dietary awareness in the research group ( P < 0.05 ). There exhibited no significant difference in the score of life quality before nursing ( P > 0.05 ); after nursing, the score of life quality lessened, and the scores of physiological function, psychological function, social function, and health self-cognition in the research group were lower ( P < 0.05 ). Conclusion. The personalized health management model based on the Internet mode can effectively enhance the chronic disease physiological indexes such as blood pressure, blood lipids, and blood sugar and improve the self-management ability of the patients with chronic disease, which is worth further popularizing and applying in the clinical physical examination in clinic.

Effectiveness of Instructional Design Framework Based on Cognitive Load Theory for Clinical Skills Training

Purpose: Cognitive load theory (CLT) is receiving increased recognition in medical education and it was cited as an important theoretical framework for simulation-based medical education. Simulated learning environments can place a high demand on the cognitive resources of the learners, hence, we aimed to design an instructional framework to optimise the total cognitive load imposed on the medical students during their clinical skills training in the clinical skills laboratory. Method: This study is a quasi-experimental post-test design. The sampling technique was purposive sampling, which included year 2 students at the Faculty of Medicine-Suez Canal University population. The study was conducted in the clinical skills and simulation laboratory. The intervention group received a developed instructional design framework based on CLT. The control group learned with the ordinary teaching method without any intervention. The cognitive load was measured using the Cognitive Load Inventory (CLI) immediately after the training session for both groups. Furthermore, students’ achievement in the clinical skill laboratory was compared in both groups. Findings: The total cognitive load is lower in the group that received the developed instructional design than that of the control group, and this result was statistically significant. Also, the performance of the intervention group is higher than in the control group. Implications for research and practice: The developed instructional design framework is a potentially useful guide for managing students' cognitive load in the clinical skills training session.
