The Surgical Results of Onlay Mesh Repair for Incisional Hernia

Abstract: Anahtar Kelimelerİnsizyonel Herni; Onlay Mesh Tamiri; Nüks Abstract Aim: İncisional hernia after abdominal surgery is an important problem. We aimed to evaluate the long-term recurrence rate as well as surgical complications in patients operated with onlay mesh repair technique for incisional hernia. Material and Method: We studied a serial of 139 patients retrospectively, operated due to incisional hernia in between January 2001 to November 2009 in Vakıf Gureba Training and Research Hospital General Surgery D… Show more

“…Seroma collection is an inflammatory response[ 16 17 ] which intensity depends on the mesh-tissue area of contact; it is minimal with inguinal hernioplasty because the implanted mesh dimension is small and tissue dissection is minimal. Much of the seroma gets absorbed and does not require intraoperative placement of a drain but in the event of its occurrence postoperatively can be adequately treated by aseptic aspiration.…”

“…That most patients in our series presented with recurrent hernias within six months of a previous suture repair strongly suggests technical failure of such repairs; late recurrence occurs at five years and is usually caused by defective healing. 3 22 23 Suture repair of incisional hernia is still practiced 24 but often leads to fascial tension with the suture eventually cutting through the fascia and ultimately leading to recurrence rates of 30-50%. It was our observation that all the failed repairs did not show evidence of having dissected and employing the fascial layer.…”

“…10 Open incisional hernia repair with mesh are prone to developed wound complication like seroma formation and wound infection leading to increased morbidity. 11,12 This study aimed to compare the outcomes between two popular methods of mesh placement technique (onlay vs. sublay) in the patients who underwent incisional hernia repair in the local setup.…”
