pontaneous rupture of the abdominal wall hernia is very rare and usually occurs in an incisional and recurrent groin hernia [1]. Despite our awareness of many causative factors and improvements in the surgical techniques and materials, iatrogenic abdominal wall hernia formation remains a significant problem in an abdominal wall surgery. An incisional hernia is known to occur in 11-23% of laparotomies [2].Commoner complications of such hernias include pain, discomfort, skin changes, bowel obstruction, incarceration, and strangulation. These tend to reduce the overall quality of life. Spontaneous rupture with the evisceration of a gut in an incisional hernia is very rare but represents a real surgical emergency. These hernias are of particular concern for their high recurrence rates and the challenges associated with their repair [3]. We report rupture of a large incisional hernia with the evisceration of small bowel in a 56-year-old female.

This surgical emergency was successfully managed using the current standard tension-free prosthetic mesh.
