The StarClose Vascular Closure Device in Antegrade and Retrograde Punctures: A Single-Center Experience

Abstract: The StarClose device is associated with a low major complication rate. A higher rate of minor complications was observed following antegrade punctures but all were managed with simple compression. Prospective randomized trials comparing closure devices are needed to evaluate their relative efficacy and safety in antegrade and retrograde punctures.

“…Apart from a handful of studies, reports detailing experience with the StarClose device during peripheral angioplasty procedures have been few. [8][9][10][11] Thus, we performed a retrospective study that spanned 5 years and .1000 consecutive peripheral angioplasty procedures with either retrograde or antegrade femoral artery catheterization.…”

“…An explanation might be that femoral angiography was performed to exclude severe stenosis, severe calcification, dissection, distortion or vascular malformations before IABP support procedures. In other studies [2,4,5,6,7,18,19,20,22,23,24], the StarClose device was used for hemostasis immediately after diagnostic and interventional catheterizations whereas in our study, the procedure was performed 2–7 days after IABP insertion. The elasticity of 8-Fr holes was poor, and the blood exuded easily through the puncture channel after deployment of the StarClose, which might be the reason for the higher rate of bleeding complications in our series.…”

“…Glycoprotein IIb/IIIa inhibitors were utilized in 19.0% of patients. However, the patients undergoing diagnostic catheterization only took aspirin [7,8,24]. Anticoagulation treatment was not routinely used after peripheral angioplasty procedures [6,19,20].…”

“…But this problem was also found in heavily calcified vessels and previous surgery. The clip-based closure system Star Close was used by Chiu et al 10 in 142 cases. In 11/142 (7.7%) patients, the system could not be successfully placed, especially with the antegrade puncture technique.…”
