Halo naevus cells on frozen sections react with sera from patients with benign halo naevus or with malignant melanoma. Halo naevus cells also react with our specifically absorbed heterologous antihuman malignant melanoma antiserum (RAMMA). Since the cytoplasm of all the naevus cells within the halo naevus react with sera from patients with malignant melanoma, this implies that the entire population of melanocytic cells in the mole has undergone mutation, because non-pathological skin melanocytes do not react.

Melanoma specific protein: occurrence in the urine of patients with halo naevus and vitiligo

Melanoma specific protein is immunologically related to altered naevus cell cytoplasm. It is excreted by patients with malignant melanoma but in no other malignancy. The protein has been detected in patients with actively developing halo naevi but not when repigmentation is taking place. It also occurs in patients with very active vitiligo but in no other pigment condition we have studied. It is suggested that the protein is a marker of active destruction of naevus cells by immune mechanisms and that the release of toxic materials during this immune reaction may be responsible for the production of the halo phenomenon and for the areas of vitiligo that may be seen elsewhere on the skin.

The Immunology of Malignant Melanoma. Summing'. A survey of the results of immunologic investigations in tumors concerning the malignant melanoma, is given -subdivided into the various in vitro and in vivo tests-, and emphasizing the viewpoints related to the clinic.Antibodies against various structures of melanoma cells can be traced optically by fluorescence in patients with melanoma but also in subjects with halo naevus. The motility of white blood cells of patients suffering from melanoma is inhibited by extracts of melanoma cells, but mostly only in localized tumors, as cytotoxic antibodies can be found only in such cases. The identification of cytotoxic lymphocytes and the stimulation of lymphocytes of patients through extracts of melanoma or PHA also depends essentially on the stage of the tumor. The serum factors-blocking the lymphocytes function-predominantly occur in the late stage of the tumor. The size of the lymphocytic stroma-reaction seems to be important regarding the prognosis. It is uncertain whether other serum factors play a decisive role in melanoma. Including unimportant restrictions, serogenetic and HL-A-investigations did not reveal obvious findings. Also is it uncertain in melanoma whether viruses play a part.By means of intracutaneous tests with unspecific and specific antigens, one tries to abtain in vivo information about the immunologic defence condition of the patient, thus one tries to ameliorate that condition with unspecific antigens or various preparations of melanoma cells. So far, all kinds of immunotherapy in melanoma resulted-if any-only in temporary successes. Unter vorwiegend klinikbezogenen Gesichtspunkten werden die tumorimmunologischen Ergebnisse beim malignen Melanom, aufgeschlfisselt nach den verschiedenen in vitround in vivo-Tests zusammengefagt.Antik6rper gegen verschiedene Strukturen der Melanomzellen k6nnen bei Melanompatienten aber auch bei Personen mit Naevus Sutton fluorescenzoptisch nacbgewiesen werden. Weige Blutzellen yon Melanompatienten werden durch Melanomzellextrakte in ihrer Beweglichkeit gehemmt, meistens jedoch nur bei lokalisiertem Tumor, wie auch cytotoxische Antik6rper nur in solchen F/illen zu finden sin& Der Nachweis yon cytotoxischen Lymphocyten und die Stimutierbarkeit yon Pafienten-Lymphocyten durch Melanom-Extrakte oder PHA b/ingt ebenfatls wesentlich vom Tumor-Stadium ab. Die Lymphocytenfunktion blockierende Serumfaktoren treten vorwiegend im fortgeschrittenen Tumorstadium auf. Die Stfirke der lymphocytfiren Stromareaktion scheint prognostisch bedeutsam zu sein. Es ist ungewfl3, ob andere Serumfaktoren eine entscheidende Rolle bei Melanomen spMen. Serogenetische und HL-A-Untersuchungen baben mit geringen Einschrfinkungen zu keinen aufffilligen Befunden geffihrt.
