The sonographic prediction of invasive carcinoma in unilocular-solid ovarian cysts in premenopausal patients: a pilot study

Abstract: Transvaginal ultrasound examination seems to be able to discriminate between invasive and non-invasive tumors in the premenopausal patients with unilocular-solid adnexal masses. Because of the retrospective nature of the study, further prospective clinical trials are needed to confirm the accuracy of the selected sonographic parameters in discriminating the invasive and non-invasive adnexal tumors.

“…To the best of our knowledge there is only one published study reporting which clinical and ultrasound variables can be used to distinguish benign from malignant unilocular‐solid cysts with papillations but no other solid components. Two other studies investigated the possibility of using the sonographic appearance of papillations to discriminate between benign and malignant tumors, but one of them included unilocular‐solid cysts not only with papillations but also with other solid components and the other study also included lesions containing both septa and solid components other than papillations. The details of these three studies are summarized in Table .…”

“…These were defined a priori by members of the IOTA Steering Committee (D.T., L.V., A.T., T.B.) and the principal investigators in the seven participating centers, both according to their personal experience/judgement and based on evidence from current scientific literature on the subject. Information on these additional ultrasound features was obtained by a single investigator (C.L.…”

“…The preoperative detection of SSPBOT could have a considerable positive effect on the management of the disease in younger women in whom the preservation of reproductive capacity is critical . At surgery, SSPBOTs tend to appear as solid, irregular lesions that are indistinguishable from invasive epithelial and metastatic tumors (Figure ).…”

“…The preoperative detection of SSPBOT could have a considerable positive effect on the management of the disease in younger women in whom the preservation of reproductive capacity is critical. 16,17 At surgery, SSPBOTs tend to appear as solid, irregular lesions that are indistinguishable from invasive epithelial and metastatic tumors (Figure 2). Prior knowledge of the likely tumor type and the presence of normal ovaries-or at least some normal ovarian tissue-in its center is likely to facilitate attempts to conserve the ovaries at surgery.…”

“…Surgical management of very small ovarian lesions is a challenge, particularly in young women with childbearing potential. The surgical strategy may be conservative cystectomy (worsening the prognosis owing to risk of spillage if the tumor is malignant, and causing harm by damaging the ovarian reserve if the tumor is benign) or adnexectomy (with the risk of overtreatment in patients with a benign tumor).…”
