Introducción: la cirugía de Fontan, realizada por primera vez en 1968 como paliación para la atresia tricuspídea, está indicada para cardiopatías con fisiología univentricular. Consiste en drenar la totalidad de la sangre caval hacia las arterias pulmonares, ya sea por túnel interatrial o extracardíaco, al cual se le realiza una fenestra que drena hacia el atrio pulmonar. La preparación previa, mediante cirugías como la fístula sistémico-pulmonar o el procedimiento de Glenn, es crucial al conseguir un adecuado lecho vascular pulmonar y evitar el fracaso del sistema. Objetivo: comunicar los primeros casos de Fontan en el Centenario Hospital Miguel Hidalgo y evaluar los resultados obtenidos a corto plazo. Metodología: Estudio observacional, descriptivo y retrospectivo. Se revisaron los expedientes de los pacientes con fisiología univentricular desde 2000 a 2018. Resultados: la cirugía de Fontan se ha realizado desde 2015, utilizando un protocolo en tres etapas. Se han operado a 3 hombres y 2 mujeres, el diagnóstico más frecuente fue la atresia tricuspídea. Todos se sometieron a evaluación ecocardiográfica y cateterismo previo. El procedimiento se realiza con asistencia ventricular y sin pinzamiento aórtico, tipo túnel extracardíaco con injerto de politetrafluoroetileno y fenestra laterolateral a atrio derecho. La mortalidad inmediata fue de 2 pacientes por choque cardiogénico y séptico. Las complicaciones tempranas más frecuentes fueron el derrame pleural (n=3/5) y la hemorragia (n=2/5). A todos se les ofrece tromboprofilaxis mediante antiagregación plaquetaria, sin reportarse eventos tromboembólicos. Conclusión: la mortalidad reportada no se considera como absoluta en el centro, ya que el comienzo de la realización de esta cirugía es reciente y el volumen pequeño. Por la complejidad del procedimiento se someterá a evaluación constante y rigurosa de los resultados obtenidos y a comparación de técnicas y desenlaces en otros grupos nacionales e internacionales con el fin de mejorar nuestro desempeño y calidad de la atención. . We report two cases of successful Fontan operation in children with heterotaxy syndrome associated with univentricular physiology and absent and nonconfluent central pulmonary arteries with both distal pulmonary arteries directly connected to the ipsilateral ductus arteriosus. After unilateral systemic-pulmonary shunt, the central pulmonary artery was reconstructed with a polytetrafluoroethylene prosthetic graft concomitantly with bidirectional cavopulmonary shunt. Finally, extracardiac total cavopulmonary connection was performed as an off-pump procedure. Children with bilateral ductus arteriosus and a nonconfluent pulmonary artery with univentricular physiology present a particular challenge in regard to completing Fontan operations. Careful attention should be directed at ensuring balanced growth of the bilateral distal pulmonary arteries. When planning reconstruction of the central pulmonary artery with a prosthesis, late reconstruction may be beneficial, as it enables utilization of a larger-caliber graft, obviating the need for replacement during a subsequent Fontan operation.
