The Societe Francaise d'Oncologie Pediatrique LMB89 protocol: highly effective multiagent chemotherapy tailored to the tumor burden and initial response in 561 unselected children with B-cell lymphomas and L3 leukemia

Abstract: Five-year survival is 92.5% (95% confidence interval [CI], 90%-94%) and eventfree survival (EFS) 91% (95% CI, 89%-93%). EFS is 98% (95% CI, 90%-100%), 92% (95% CI, 89%-95%), and 84% (95% CI, 77%-90%) for group A, B, and C, respectively. In group B, multivariate analysis of prognostic factors showed that a lactate dehydrogenase level more than 2-fold the normal value, no response after COP, and age of at least 15 years were associated with a lower EFS. CNS involvement was the only prognostic factor in group C. … Show more

“…Previous study showed a more favorable prognosis of DLBCL in pediatric than adult patients [1,[6][7][8]. In the current study also, the young group had a better event-free and overall survival than the older group, although the difference was not significant.…”

“…On the basis of the records of physical examinations, surgical notes, and pathologic examinations of the specimens, the Ann Arbor staging system was applied in all 81 patients. The IPI score was calculated with four adverse factors (age > 60 years, Ann Arbor Stages III and IV, Eastern Cooperative Oncology Group performance score 2-4, and serum LDH elevation) present at the time of diagnosis [6]. For patients under 60 years, stage, performance score, and LDH elevation were considered as adverse factors [9].…”

“…All patients were HIV-negative. Treatment protocols included: the institutional B-cell lymphoma protocol (SJBCII), a modification of the LMB-89 protocol 14 (12 patients), the Pediatric Oncology Group (POG) 8617 protocol 24 (13 patients), and various other protocols (8 patients). The tumor samples included bone marrow aspirates (20 patients) and other tissues (13 patients).…”

“…11 With the current chemotherapy regimens, the overall cure rates are approximately 90% in children and 50% to 60% in adults. [12][13][14][15] However, patients with high-stage disease continue to have a less favorable prognosis, with cure rates of 80% to 85% in children 15 and 40% to 50% in adults 16 There are no known biological tumor features that reliably predict the behavior of BL. In addition, it is not entirely clear if the difference in outcome between children and adults is a result of a relatively poor tolerance to treatment in the latter or if it is at least partially attributable to differences in tumor biology.…”
