Background: Quality of life (QOL) has been identified as an important outcome in cancer researches yet the most common malignancy among humans, non-melanoma skin cancer (NMSC), but poorly studied. Aim of the study was to analyze the quality of life of non-melanoma skin cancer patients after surgery using dermatology life quality index inventory (DLQI). Methods: Retrospective cohort of patients operated for non-melanoma skin cancer in last 2 years and paid postoperative 4-month visit formed the study population. Inclusion criteria consisted of subjects operated for nonmelanoma skin cancer and paid follow up visits having sufficient physical and mental capacity. Fifty-six subjects fulfilled the selection criteria laid down thus included in this study. Study tools were records of patients, which were obtained from medical records section. If any more information was required, study subjects were contacted. Results: Out of total 56 study subjects, Basal cell carcinomas were found in 91.1% (n=51); squamous cell carcinomas were detected in 7.2% (n=4). Single location wise more lesions were located on the nose 22.1% (n=15) and forehead 17.6% (12). For most patients (75%), the lesion had not been previously treated. 58.9% subjects did not have any other associated co-morbid condition. Lower mean values were observed post-operative i.e. lower DLQI scores were recorded 4 months after surgery in our study which indicates that adverse effects were not very prominent thus preserving quality of life post operatively. Paired sample t-tests revealed a significant effect on DLQI item 1 (p=0.008), item 2 (p=0.043), and item 4 (p=0.003), with scores decreasing (improving QOL) after treatment. The change in total DLQI score demonstrated a trend toward significance, with overall QOL improving after treatment (p=0.024). Conclusions: Previously commonly employed dermatological Quality of life tools demonstrated minimal handicap at initial diagnosis and little change after treatment of nonmelanoma skin cancer. Development of disease-specific instrument is warranted to explore the disease process.

Instrumenti kvaliteta života specifični za dermatologiju, sve češće se primenjuju u istraživanju kod pacijenata sa kožnim bolestima. CIlj je ispitati odnos ocena kvaliteta života dobijenih primenom dva za dermatologiju specifična instrumenta. Istraživanjem je obuhvaćeno 62 ispitanika sa kožnim bolestima, kod kojih je kvalitet života ocenjen primenom instrumenata specifičnih za dermatologiju (tj. Dermatology Life Quality Index-DLQI i Skindex-16), namenjenih upotrebi kod svih kožnih bolesti. Ukupni skorovi DLQI i Skindex-16 nalazili su se u jakoj korelaciji (r=0.87; p<0.001). Postojale su i statistički značajne korelacije ukupnog skora DLQI sa skorovima za podskale simptomi, emocije i funkcionisanje iz r=0.77 i r=0.86, redom). Ukupan skor Skindex-16 nije bio u korelaciji sa skorom za podskalu tretman iz DLQI-a, međutim, bio je u korelaciji sa skorovima za podskale simptomi i osećanja, dnevne aktivnosti, slobodne aktivnosti, rad ili škola i lični odnosi iz r=0.80, r=0.66, r=0.62 i r=0.68, redom). Dva instrumenta specifična za dermatologiju (npr. DLQI i Skindex-16), ukoliko se zajedno primenjuju, mogu pružiti komplementarne podatke pri ocenjivanju kvaliteta života pacijenata sa kožnim bolestima.Ključne reči: Kožne bolesti, kvalitet života, DLQI, Skindex-16. UVODOboljenja kože u retkim slučajevima ugrožavaju život, međutim, mogu imati nepovoljan uticaj na sve aspekte kvaliteta života, kao što su psihološka dimenzija, fizičko i socijalno funkcionisanje [1,2].Za ocenu kvaliteta života pacijenata sa kožnim bolestima, konstruisano je nekoliko instrumenata specifičnih za dermatologiju, uključujući Skindex-16 i DLQI [3,4,5,6]. Svaki od tih instrumenata sastavljen je od upitnika, sa različitim pitanjima. U istraživanju, instrumenti kvaliteta života specifični za dermatologiju, korišćeni su kod pacijenata sa jednim oboljenjem kože ili uopšte sa kožnim bolestima [7,8,9,10,11,12]. CILJ RADACilj ove studije bio je da ispita odnos ocena kvaliteta života dobijenih primenom dva za dermatologiju specifična instrumenta (DLQI i Skindex-16). MATERIJAL I METODEStudijska populacija uključivala je ispitanike sa ši-rokim spektrom kožnih bolesti. Svi ispitanici lečeni su na Kožnom odeljenju Zdravstvenog centra u Kosovskoj Mitrovici. Istraživanjem je obuhvaćeno 62 ispitanika. Ispitanici mlađi od 18 godina i ispitanici sa mentalnim i telesnim bolestima nisu uključeni u studiju. BackgroundHyperhidrosis is uncontrollable excessive sweating that occurs at rest, regardless of temperature. The symptoms of hyperhidrosis can significantly affect quality of life. The management of hyperhidrosis is uncertain and variable.ObjectiveTo establish the expected value of undertaking additional research to determine the most effective interventions for the management of refractory primary hyperhidrosis in secondary care.MethodsA systematic review and economic model, including a value-of-information (VOI) analysis. Treatments to be prescribed by dermatologists and minor surgical treatments for hyperhidrosis of the hands, feet and axillae were reviewed; as endoscopic thoracic sympathectomy (ETS) is incontestably an end-of-line treatment, it was not reviewed further. Fifteen databases (e.g. CENTRAL, PubMed and PsycINFO), conference proceedings and trial registers were searched from inception to July 2016. Systematic review methods were followed. Pairwise meta-analyses were conducted for comparisons between botulinum toxin (BTX) injections and placebo for axillary hyperhidrosis, but otherwise, owing to evidence limitations, data were synthesised narratively. A decision-analytic model assessed the cost-effectiveness and VOI of five treatments (iontophoresis, medication, BTX, curettage, ETS) in 64 different sequences for axillary hyperhidrosis only.Results and conclusionsFifty studies were included in the effectiveness review: 32 randomised controlled trials (RCTs), 17 non-RCTs and one large prospective case series. Most studies were small, rated as having a high risk of bias and poorly reported. The interventions assessed in the review were iontophoresis, BTX, anticholinergic medications, curettage and newer energy-based technologies that damage the sweat gland (e.g. laser, microwave). There is moderate-quality evidence of a large statistically significant effect of BTX on axillary hyperhidrosis symptoms, compared with placebo. There was weak but consistent evidence for iontophoresis for palmar hyperhidrosis. Evidence for other interventions was of low or very low quality. For axillary hyperhidrosis cost-effectiveness results indicated that iontophoresis, BTX, medication, curettage and ETS was the most cost-effective sequence (probability 0.8), with an incremental cost-effectiveness ratio of £9304 per quality-adjusted life-year. Uncertainty associated with study bias was not reflected in the economic results. Patients and clinicians attending an end-of-project workshop were satisfied with the sequence of treatments for axillary hyperhidrosis identified as being cost-effective. All patient advisors considered that the Hyperhidrosis Quality of Life Index was superior to other tools commonly used in hyperhidrosis research for assessing quality of life.LimitationsThe evidence for the clinical effectiveness and safety of second-line treatments for primary hyperhidrosis is limited. This meant that there was insufficient evidence to draw conclusions for most interventions assessed and the cost-effectiveness analysis was restricted to hyperhidrosis of the axilla.Future workBased on anecdotal evidence and inference from evidence for the axillae, participants agreed that a trial of BTX (with anaesthesia) compared with iontophoresis for palmar hyperhidrosis would be most useful. The VOI analysis indicates that further research into the effectiveness of existing medications might be worthwhile, but it is unclear that such trials are of clinical importance. Research that established a robust estimate of the annual incidence of axillary hyperhidrosis in the UK population would reduce the uncertainty in future VOI analyses.Study registrationThis study is registered as PROSPERO CRD42015027803.FundingThe National Institute for Health Research Health Technology Assessment programme.
