The Series‐parallel Circuit in the Treatment of Fulminant Hepatitis

Abstract: We developed a series-parallel treatment method for combined plasma exchange (PE) and continuous hemodiafiltration (CHDF) therapy in fulminant hepatitis. We then compared total serum bilirubin, citrate, and cytokine levels obtained by the new methods to those obtained with treatment by the single and reverse-parallel PE methods. Ten adult patients with fulminant hepatitis consented to participate. Plasma exchange was conducted 25 times by the single method (PE only), 16 times by the reverse-parallel method, an… Show more

“…showed that the clearance of cytokines such as IL‐6 and IL‐8 was about 18 mL/min (24). The removal of bilirubin in PDF was 35.6%, and in a combination of PE and CHDF it was approximately 44% (4–6,25). The removal rate in PDF was lower than that in PE and CHDF.…”

“…However, since PE is not selective, it not only removes toxins such as bilirubin, but also beneficial substances such as hepatocyte growth factor (24). Therefore, the usefulness of PE combined with CHDF treatment has been suggested (16). In the present case, our patient promptly responded to the combined PE and PMMA‐CHDF therapy.…”

“…There was no blood access difficulty such as catheter failure or coagulation in the filter during blood purification. We previously reported that even when the series‐parallel method was chosen for the combined PE and CHDF treatment, it was difficult to remove the excess citrate, which is contained in the anticoagulants in FFP, by our procedure of CHDF (1–3). The remaining amount of citrate might prevent clotting in the circuits.…”
