Amaç: Bu çalışmada Scimitar sendromu tanısı konulmuş hastalar değerlendirildi.Ça lış mapla nı:Çalışmaya Ocak 2003 -Aralık 2011 tarihleri arasın-da kliniğimizde Scimitar sendromu tanısı konulmuş ortalama ağır-lıkları 16.7±21.8 kg olan, 12 hasta (8 kız, 4 erkek; ort. yaş 4.0±4.8 yıl; dağılım 1 ay -16 yıl), alındı. Hastaların klinik bulguları, fizik muayene, telekardiyografi, ekokardiyografi, kalp kateterizasyonu ve cerahi bulguları ve izlem verileri geriye dönük olarak gözden geçirildi. Bul gu lar:On iki hastadan beşi (%42) infantil tip ve yedisi (%58) çocuk/erişkin tip Scimitar sendromu olarak değerlendirildi. İnfantil tipteki hastaların tümünde taşipne ve kalp yetersizliği bulguları varken, çocuk/erişkin tipteki hastaların birinde tekrarlayan alt solunum yolu enfeksiyonu öyküsü vardı. Ek kardiyak defektler hastaların yedisinde atriyal septal defekt, birinde aort koarktasyonu, ikisinde patent duktus arteriyozus ve birinde ventriküler septal defekt ile pulmoner atrezi idi. Bir hastada iki taraflı Scimitar sendromu vardı. İnfantil Scimitar sendromlu hastaların tümü şiddetli pulmoner hipertansiyonlu iken, çocuk/erişkin tipteki hastalardan biri hafif pulmoner hipertansiyonlu idi. Aortopulmoner kollateral arter saptanan dokuz hastadan altısında arter transkateter yolla kapatıldı. On hastaya (%83) cerrahi girişim uygulandı. İnfantil Scimitar sendromlu bir hasta ameliyat sonrası erken dönemde, ameliyat edilmeyen iki taraflı Scimitar sendromlu diğer bir hasta ise takipte akciğer enfeksiyonu nedeniyle kaybedildi.So nuç: Scimitar sendromu farklı klinik tablolarla seyredebilen nadir bir sendromdur. Çocuk/erişkin tip hastalığı olan hastalar sıklıkla asemptomatiktir; ancak tekrarlayan akciğer enfeksiyonları da izlenebilir. Aortopulmoner kollateral oklüz-yon semptomatik iyileşme sağlayabilse de özellikle Scimitar veni tıkanmış ve ilave kardiyak anomalisi bulunan hastalarda cerrahi gerekir.Anah tar söz cük ler: Ekokardiyografi; pulmoner hipertansiyon; Scimitar sendromu.Background: This study aims to evaluate patients diagnosed with Scimitar syndrome.

Scimitar syndrome is characterized by an anomalous venous return with the characteristic chest roentgenogram (CxR) appearance of the anomalous vein draining into the inferior vena cava (IVC). This appears as a curvilinear opacity paralleling the right border of the heart resembling a curved sword or Scimitar. A 27-year-old white woman with a reported history of dextrocardia was admitted after a drug overdose. Examination demonstrated an obtunded woman with tachycardia and right sided heart sounds. Her CxR revealed a right sided heart image with two curvilinear opacities in the retrocardiac area. Chest computed tomography (CT) demonstrated that these opacities join to represent an anomalous vein draining into IVC. Furthermore, an anomalous systemic artery arising from the abdominal aorta was seen to supply the right lower lobe. The patient was eventually diagnosed with Scimitar syndrome. This syndrome affects 1-3 in 100,000 live births while nearly half of the patients remain asymptomatic with some initially being misdiagnosed as dextrocardia, such as in our case. Correctly diagnosing these patients is of paramount importance as some can develop severe pulmonary hypertension and right ventricular failure. In turn, close ongoing echocardiographic monitoring can help identify those that may benefit from surgical interventions to prevent them from developing these complications.

Scimitar sendromu veya pulmoner venolober sendrom, nadir görülen bir konjenital venöz dönüş anomalisidir. Kırk yedi yaşında kadın hasta, göğüs ağrısı ve eforla gelişen nefes darlığı şikâyeti ile başvurdu. Fizik muayenesi ve laboratuvar bulguları normaldi. Postero-anterior akciğer grafisinde sağ akciğer alt alanda sağ kalp kenarı boyunca nonhomojen opasite artışı, sağ akciğerde volüm kaybı, kalp gölgesinin sağa doğru yer değiştirdiği izlendi. Ekokardiyografide sistolik pulmoner arter basıncı 55 mmHg ölçüldü. Fiberoptik bronkoskopide sağ bronşial sistemde ara bronşun olmadığı görüldü. Manyetik rezonans anjiografide, Scimitar sendromu ile uyumlu olarak sağ pulmoner venlerin subdiafragmatik mesafede inferior vena kavaya drene olduğu izlendi. A 47-year-old woman presented at our clinic with chest pain and shortness of breath during exertion. The physical examination and laboratory findings were normal. The chest radiograph demonstrated non-homogeneous increased opacity along the right cardiac border, and volume loss of the right lung with cardiac silhouette displacement, to the right. Echocardiography pulmonary artery systolic pressure was measured as 55 mmHg. Fiberoptic bronchoscopy revealed the intermediate bronchus was absent. On magnetic resonance angiography, it was observed that the right pulmonary veins drained into the inferior vena cava at a subdiaphragmatic level, consistent with Scimitar Syndrome. The case, diagnosed as Scimitar syndrome, is presented in the light of the literature.

The scimitar syndrome: clinical spectrum and surgical treatment

Abstract: Four patients required surgical treatment. Indications, diagnostic procedures and surgical management are discussed. In two patients, thrombosis and fibrosis occurred in the scimitar vein that had been reimplantated in the left atrium, necessitating pneumonectomy.
