The role of omental biopsy in endometrial cancer staging

Abstract: Omental biopsy is not part of FIGO staging for endometrial cancer. The few studies that have looked into this matter have had conflicting results. This is the largest study in terms of the number of cases studying the incidence of omental involvement in endometrioid and non-endometrioid endometrial cancer. A retrospective study assessing 248 cases of endometrial cancer with omental biopsy at the time of primary surgical treatment for endometrial cancer at the Gynaecological Oncology

“…Peritoneal cytology is performed at the provider's discretion, although it was taken out of the surgical staging system in 2009, and it has been hypothesized that positive cytology sometimes results from uterine manipulation, especially in minimally invasive surgery. Omentectomy is usually included for clear cell, serous, and sometimes carcinosarcoma histologies given an increased risk of advanced‐stage disease at diagnosis and relatively low morbidity with the procedure, although the risk of microscopic involvement in the absence of visible macroscopic disease is low …”

“…It was 9.1% in our previous study evaluating mixed-type endometrial carcinoma (19). Similarly, the ratio of omental metastasis in endometrial cancer was reported between 2.4% and 8.3% (8)(9)(10)(11)(12)(13)(14)(15). Nevertheless, in these studies, omentectomy was performed as infracolic omentectomy or omental biopsy.…”

“…Omental spread was thought to occur as a result of peritoneal seeding and regional lymphatics (11). On the other hand, the omentum is most probably a different region for metastases of endometrial tumor cells compared to other regions in the body.…”
