The role of mesenchymal stem cells in hematopoietic stem cell transplantation: prevention and treatment of graft-versus-host disease

Abstract: Background The use and effectiveness of hematopoietic stem cell transplantation (HSCT) are limited by lethal complications, i.e., acute and chronic graft-versus-host disease (aGVHD and cGVHD, respectively), in which immune cells from the donor attack healthy recipient tissues. GVHD presents both prophylactic and therapeutic challenges, and overall survival is poor. Mesenchymal stem cells (MSCs) show considerable promise in the treatment of GVHD because of their potential immunomodulatory activity.… Show more

“…Advances in cell therapy in recent years are associated with the use of the immunosuppressive properties of MSCs in transplantology, oncology, and some other areas of medicine, although many issues remain unresolved [46]. Depending on the microenvironment, MSCs can exhibit both immunosuppressive and immunostimulating properties [47,48].…”

“…Recently, a meta-analysis has shown that BMSCs can significantly reduce the incidence rate of chronic GVHD but not acute GVHD in patients. To prevent GVHD, it is important to use BMSCs after HSCs implant to minimize the side effects of stem cell therapy [135]. Besides, BMSCs also applied to treat chronic inflammatory diseases [136,137].…”

“…Hematopoietic stem cell transplantation (HSCT), either autologous or allogeneic, is an intensive therapy for patients who suffer from refractory or relapsing neoplastic disease and non-neoplastic genetic disorders 1,2 . Global, the rate of allogeneic HSCT continues to increase such that about 50,000-60,000 transplantation was performed annually.…”

“…In HSCT success is limited by two main factors: transplant-related morbidity or mortality (TRM) and disease relapse 4 . Graft-versus-host disease (GVHD) is the major cause of TRM which classified as either acute GVHD (aGVHD) or chronic GVHD (cGVHD) 2 . Clearly, GVHD is a common serious complication of HSCT in which immune cells from the donor attack recipient non-hematopoietic tissues, containing the gastrointestinal tract, liver, skin, and lungs 5 .…”
