The role of alcohol consumption in future classifications of alcohol use disorders

Abstract: Background-Item response theory (IRT) was used to determine whether DSM-IV alcohol abuse and dependence and consumption criteria were arrayed along a continuum of severity.Methods-Data came from a large, nationally representative sample of the U.S. adult population.Results-DSM-IV alcohol abuse and dependence criteria formed a continuum of alcohol use disorder severity along with the drinking 5+/4+ at least once a week in the past year criterion. Criteria were invariant across sex, race-ethnicity, and age subgr… Show more

“…Among the dependence symptoms, the larger/longer criterion appeared to be a particularly effective item for discriminating between those with heavy drinking and no AUD symptoms and those with at least some AUD symptoms. This is consistent with population-based studies in the U.S. (Saha et al, 2007) and Australia (Proudfoot et al, 2006).…”

“…These results extend those of Krueger et al (2004) and Saha et al (2007) by showing that, in addition to episodic heavy drinking, both weekly drinking and regularly drinking more than the NIAAA recommended weekly limit can be used to map the less severe regions of the alcohol involvement continuum. In this sample, the chosen indices of alcohol use all were less severe than diagnostic symptoms of alcohol abuse and dependence.…”

“…However, in both men and women, those with a diagnosis of MDD were more likely to endorse individual criteria than their counterparts who did not meet criterion for past-year MDD. Note that the prevalence of these criteria in past-year regular users has also been reported in Saha et al (2006Saha et al ( , 2007.…”

“…However, in view of the high correlation between abuse and dependence in the two-factor models, many clinical investigators favor a single factor as more appropriate in treatment samples. More recent applications of item response theory in adult samples (Kahler and Strong, 2006;Krueger et al, 2004;Langenbucher et al, 2004;Saha et al, 2006Saha et al, , 2007 and treated adolescents (Martin et al, 2006) suggest that abuse and dependence criteria are intermixed along the spectrum between mild and severe aspects of symptom pathology.Several investigators (Budney, 2006;Hughes, 2006) emphasize the need for further studies of the validity and utility of generic versus drug-specifi c criteria for substance use disorders. Clinical studies need to be supplemented with general population samples.…”
