Background:Adrenal incidentaloma is an adrenal neoplasm frequently encountered in clinical practice for which detection rates have recently increased. We describe here the clinical characteristics of adrenal incidentalomas. Methods: A retrospective study was performed examining the age, sex, location, size, function, and the histological findings for 348 patients with an adrenal mass discovered incidentally on computed tomography (CT) undertaken for health examination or nonadrenal disease from August 2005 to May 2012. Results: Patients consisted of 156 males (44.8%) and 192 females (55.2%), aged between 20 and 86. Adrenal masses were most commonly found in patients in their sixth decade (32.5%). Regarding the location of the masses, 62.0% were found in the left adrenal gland, 30.2% were found in the right, and 7.8% were found bilaterally. Of all of the masses analyzed, 87.1% were 1 to 4 cm in size, and an adenoma-like appearance was the most common finding (75.3%) seen on CT scans. Hormonal analysis showed that 82.2% of the masses were nonfunctioning, 6.0% were diagnosed as subclinical Cushing's syndrome, 4.6% were aldosteroneproducing adenomas, and 7.2% were pheochromocytomas. Adrenalectomy was performed in a total of 69 patients having adenoma (50.7%), pheochromocytoma (24.6%), and carcinoma (4.3%). Conclusion:The characteristics of benign, malignant, nonfunctional, and functional adrenal masses that were incidentally found at our hospital were similar to those presented in other studies.

In the operation of any SPECT/CT system, in addition to internal radiation exposure (g-ray) resulting from administration of radiopharmaceuticals, external radiation exposure (x-ray) from the CT device has to be taken into consideration in the light of recommendations from the International Commission on Radiological Protection. These recommendations include justification of practices (the use of radiation produces sufficient benefit to offset any risks caused by the use of radiation), optimization (the incurred exposure by the use of radiation should be kept as low as reasonably achievable), and dose limitation. The internal radiation exposures of each organ after administration of radiopharmaceuticals are calculated by the MIRD Committee method. For example, the internal radiation exposure index for brain perfusion scintigraphy is 0.8 mGy/37 MBq for N-isopropyl-4-iodoamphetamine( 123 I) hydrochloride or 0.19 mGy/37 MBq for ethyl cysteinate dimer. On the other hand, the external radiation exposure from a CT device is calculated using the CT dose index volume (CTDIvol)-a measured and calculated value unique to the CT scanner and scan parameters used-and a dose-length product, which is obtained from CT conditions and generally used as a reference value for CT radiation exposure. However, CTDIvol and dose-length product are calculated values unique to each device, not the value of external radiation exposures of each organ. Therefore, we believe that it is necessary to measure the total (internal plus external) radiation exposure dose from CT. In the present study, using an anthropomorphic phantom for deep-body total absorbed dose measurement, we evaluated the radiation exposure doses (organ-absorbed doses) of each organ under various CT conditions. Methods: The radiation exposure doses of each organ were measured by inserting thermoluminescent dosimeter elements into the phantom under various CT conditions. Results: The following were brain radiation exposure doses in the head region. For 90 kVp and 25 mAs, 1.39 mGy (CTDIvol, 1.8 mGy), for 90 kVp and 300 mAs, 17.00 mGy (CTDIvol, 21.2 mGy), for 120 kVp and 25 mAs, 3.21 mGy (CTDIvol, 3.8 mGy), for 120 kVp and 300 mAs, 37.79 mGy (CTDIvol, 47.7 mGy), for 140 kVp and 25 mAs, 5.08 mGy (CTDIvol, 5.5 mGy), and for 140 kVp and 300 mAs, 65.07 mGy (CTDIvol, 65.6 mGy). The eye radiation exposure doses were as follows. For 90 kVp and 25 mAs, 1.94 mGy (CTDIvol, 1.8 mGy), for 90 kVp and 300 mAs, 20.31 mGy (CTDIvol,21.2 mGy), for 120 kVp and 25 mAs, 3.71 mGy (CTDIvol, 3.8 mGy), for 120 kVp and 300 mAs, 49.72 mGy (CTDIvol, 47.7 mGy), for 140 kVp and 25 mAs, 5.44 mGy (CTDIvol, 5.5 mGy), and for 140 kVp and 300 mAs, 69.76 mGy (CTDIvol, 65.6 mGy). In addition, the radiation exposure doses of the cervical, thoracic, abdominal, and pelvic regions were measured in detail. 90Адренокортикальный рак (АКР) -редкое заболевание, для которого характерно агрессивное клиническое течение и небла-гоприятный прогноз. АКР может быть гормонально-активным, являясь причиной субклинического или манифестного синдрома Иценко-Кушинга (СИК), и/или вирильного синдрома, или слу-чайно выявленной гормонально-неактивной опухоли надпочеч-ника. Доля АКР в структуре онкологической заболеваемости со-ставляет 0,04-0,2% при ежегодной заболеваемости 0,5-5,0 слу-чаев на 100 000 населения [1].Описано несколько наследственных синдромов, компонен-том которых часто является АКР, таких как синдром Ли-Фрау-мени, синдром Гарднера, синдром МЭН-1 [2]. При генетическом анализе ДНК опухолей надпочечников при спорадическом АКР выявлена структурная делеция в 17p и структурная перестановка 11p15, которые были облигатно связаны со злокачественной структурой опухоли [3]. Диагностика гормональной активностиОпухоль надпочечника чаще является случайной находкой при радиологическом исследовании. По данным компьютерной томографии (КТ) случайные образования надпочечника выявля-ются приблизительно у 4% обследованных пациентов [4]. В воз-расте до 30 лет инциденталома встречается приблизительно у 0,2% обследованных, но в группе пациентов старше 70 лет часто-та возрастает до 7%. По данным сводной аутопсийной статисти-ки, распространенность случайно выявленных опухолей надпо-чечника составляет в среднем 6%.Для случайно выявленных при радиологическом исследова-нии опухолей надпочечников диаметром более 1 см независимо от их гистологической структуры принят собирательный термин «инциденталома» [5].Представления о частоте выявления АКР при инцидентало-мах пересмотрены в связи с расширением клинической базы ис-следований и требованиями к характеру и дизайну эпидемиоло-гических исследований. Так, по данным W. Young и соавт.[6], частоту АКР оценивали чуть более 4%. Позднее межклинический анализ [7] большого числа пациентов продемонстрировал рас-пространенность АКР среди инциденталом не более 1,9%.В связи с увеличивающейся распространенностью и доступ-ностью методов топической диагностики, начиная с двух послед-них декад прошлого века отмечается увеличение количества ин-циденталом надпочечника. Это привело к увеличению удельного Адренокортикальный рак (АКр) -редкое заболевание, для которого характерно агрессивное клиническое течение и не-благоприятный прогноз. Доля АКр в структуре онкологической заболеваемости составляет 0,04-0,2% при ежегодной заболеваемости 0,5-5,0 случаев на 100 000 населения. редкость АКр обусловливает объективные трудности в диагно-стике данного заболевания из-за отсутствия адекватных проспективных исследований и достаточного опыта лечения вне специализированных учреждений, концентрирующих этих больных. представлены диагностические методы, позволяющие выработать наиболее подходящий план лечения больных АКр.
