The risk of intrapartum/neonatal mortality and morbidity following birth at 37 weeks of gestation: a nationwide cohort study

Abstract: Objective To assess intrapartum/neonatal mortality and morbidity risk in infants born at 37 weeks of gestation compared with infants born at 39–41 weeks of gestation. Design Nationwide cohort study. Setting The Netherlands. Population A total of 755 198 women delivering at term of a singleton without congenital malformations during 2010–14. Methods We used data from the national perinatal registry (PERINED). Analysis was performed with logistic regression and stratification for the way labour started and type … Show more

“…Our research reflects worse average values in the biochemical parameters of newborns at birth, and a higher percentage of acidosis in the normal gestational age range (37 to 41 gestational weeks inclusive) compared with the abnormal gestational age range (<37 or >41 weeks). This aligns with authors such as Parikh et al ( 18 ) and Sandström et al ( 19 ), with an increased rate of admission to intensive care units and higher respiratory morbidity in cases lying closer to gestational week 37, Eskes et al ( 20 ), Little et al ( 21 ), and Yi et al ( 17 ) reaffirm that birth at 37 gestational weeks is associated with a higher frequency of clinically relevant adverse perinatal outcomes. Term babies are more susceptible to mechanical injury and progressive hypoxic insults associated with prolonged and obstructed labor, which may often warrant operative intervention ( 17 ).…”

“…Of the 312 infants constituting the sample, 30.7% (96) presented acidotic pH of the umbilical artery at birth, 79.2% (76) showed respiratory acidosis, and 20.8% (20) showed mixed acidosis (Table 1).…”

“…17 Delivery at or after 38-39 weeks' gestation carries the same risk of intrapartum stillbirth and neonatal death as delivery at a later week of gestation. 17,198 These epidemiological observations underlie the 67% reduction in the risk of perinatal death associated with IOL at term. 16…”

“…Child factors included child sex (male/female), gestational age (weeks) and birthweight (grams). Standard cutoffs for prematurity were applied to gestational age (≥ 37 weeks/<37 weeks) and birthweight (≥ 2500 g/<2500 g) as per clinical standards of falling within the 10th percentile of pregnancies, with increased infant mortality risk [ 29 , 30 ]. Maternal and socioeconomic factors included maternal age at birth, maternal education, maternal history of smoking, maternal psychiatric history, and deprivation indices.…”
