The reliability, validity and factorial structure of the Swahili version of the 7-item generalized anxiety disorder scale (GAD-7) among adults living with HIV from Kilifi, Kenya

Abstract: Background Generalized Anxiety Disorder (GAD) is under-investigated in people living with HIV/AIDS from sub-Saharan Africa. In part, this is due to paucity of culturally appropriate measures for GAD which are psychometrically robust. This study aimed to evaluate the reliability, factorial structure, and validity of Swahili version of the 7-item GAD questionnaire (GAD-7) among adults living with HIV. Study design Descriptive cross-sec… Show more

“…The recommended optimal cut-off score of ≥10 for both PHQ-9 and GAD-7 [42,43], also applicable in sub-Saharan Africa [44,45], was used to define positive screen for depressive and anxiety symptoms. PHQ-9 and GAD-7 have locally been validated, showing good psychometric properties [46,47]. In the present study, the internal consistency alphas for PHQ-9 and GAD-7 were all good, 0.83 (95% CI 0.81, 0.86), and 0.86 (95% CI 0.84, 0.88), respectively.…”

“…The GAD‐7 has demonstrated strong psychometric properties in validation studies including excellent internal consistency (Cronbach’s α = 0.92) and strong test–retest reliability (r = 0.83). It has been validated among Kenyan HIV/AIDS populations (α = 0.82), with acceptable test–retest reliability (ICC = 0.70) [33].…”

“…For anxiety we used Generalized Anxiety Disorder 7 (GAD 7) which we scored 0–4 = no anxiety, 5–9 = mild anxiety, 10–14 = moderate anxiety and ≥15- severe anxiety ( 17 ). GAD 7 has demonstrated good internal consistency and convergent validity in heterogeneous samples ( 18 ) and has been used within Kenyan settings ( 19 , 20 ). For depression we used Patient Health Questionnaire (PHQ 9) for depression which we scored 0–5 = Mild depression, 6–10 = moderate depression, 11–15 = moderately severe and 16–20 =severe depression ( 21 ).…”

“…The mental health impact of the Corona Virus disease of 2019 (COVID- 19) is expected be huge due to the nature of the disease and the containment measures imposed (1). There is sufficient evidence to indicate that during any epidemic or pandemic, healthcare workers are affected mentally to a great extent (2).…”
