The relationship between leukemoid reaction and perinatal morbidity, mortality, and chorioamnionitis in low birth weight infants

Abstract: In low birth weight newborn infants, NLR is significantly associated with sepsis, intraventricular hemorrhage, bronchopulmonary dysplasia, and a high mortality rate. Also, those infants with NLR are more likely to be born to mothers with chorioamnionitis and they face sepsis and intraventricular hemorrhage more often.

“…Other mechanisms, such as a neonatal leukemoid reaction induced by CA, have also been suggested as important risk factor for BDP. 94,95 In contrast to HCA, Alexander et al reported an increased incidence of RDS among preterm babies exposed to CCA. Of note, many of these studies were conducted prior to the widespread use of prenatal corticosteroids for fetal lung maturation, exogenous surfactant administration, and/or noninvasive ventilation, which are currently considered standard practices with proven benefit for patients and improved respiratory outcomes.…”

“…Although the exact causative mechanism is still unclear, several hypotheses are documented in the literature, of which chorioamnionitis, 2,8,9 perinatal infection, 6,10,11 and the administration of ACS 12-14 seem to be the most important triggers.…”

“…[1][2][3][4][5][6] It is defined as a state in which the white blood cell count is Z50 Â10 9 /L or the absolute neutrophil count is Z30 Â10 9 /L during the neonatal period. 7 In this case report, we describe 2 dichorionic diamniotic premature twins, of whom the 2 girls had an extreme hyperleukocytosis, whereas their siblings did not.…”

“…Neonates with leukemoid reaction had a 4-fold increase in sepsis, 20-fold increase in intraventricular hemorrhage, 54-fold increase in bronchopulmonary dysplasia and 6-fold increase in mortality compared with controls. 16 The decision to proceed with double volume exchange transfusion in this patient was based on concerns of high blood viscosity contributing to the risk of neurological injury. This baby was at considerable risk of a poor neurodevelopmental outcome because of prematurity, low 10 min Apgar score and the prominent vesicular rash at birth.…”
