A 2014-16-os években a korábban észlelt trend folytatódott a magyarországi katéterablációs beavatkozások számá-nak alakulásában. Amíg a konvencionális ablációk számában minimális emelkedés volt észlelhető, a komplex beavatkozások és ezen belül a pulmonalis véna izolációk számának exponenciális emelkedését látjuk. Jelen közleményben közöljük a fenti években közzétett ablációs számokat és összehasonlítjuk a közép-kelet-európai adatokat valamint az egyes magyarországi centrumok teljesítményét. Numbers of cardiac electrophysiology studies and catheter ablations in Hungary between 2014-2016The trend continued from 2014 to 2016 concerning the number of catheter ablations in Hungary. While the number of conventional ablations increased slightly, there was an exponential increase in the number of complex interventions and in the number of pulmonary vein isolations. In this paper, we present the number of ablations performed in the years mentioned above and we compare the Central and Eastern European data, and the performance of each Hungarian centers.Kulcsszavak: elektrofi ziológia, katéterabláció, pitvarfi brilláció, ritmuszavar electrophysiology, catheter ablation, atrial fi brillation, arrhytmia Keywords: BevezetésA szív-elektrofi ziológiai vizsgálatok (EP) és katéterab-lá ciók száma világszerte, így Magyarországon is ugrásszerűen emelkedik. Ennek több oka is van. A rit muszavarok alaposabb megismerése, a katéterab-lációs technikák fejlődése és az utóbbi években kialakult elektrofi ziológusi tréningrendszerek a katéterab-lációs beavatkozások szélesebb körű elterjedését és gyakoribb alkalmazását hozták magukkal. Az ESC-és AHA-ajánlásokban a legtöbb ritmuszavar esetében a katéterabláció elsőként válaszható és választandó beavatkozás lett (1, 2). Ha a magyarországi helyzetet szeretnénk megvizsgálni, ezt legkönnyebben a Magyarországhoz hasonló adottságú és népességű országokkal történő összehasonlítással tehetjük meg. Magyarország a közép-kelet-európai régióbanA korábban publikált adatokkal és a környező orszá-gok adataival hasonlítottuk össze a 2014-2016-os eredményeket. SummaryAtrial fibrillation (AF) is an epidemic, affecting 1% to 1.5% of the population in the developed world. Projected data from the population-based studies suggest that the prevalence of AF will grow at least 3-fold by 2050. The health and economic burden imposed by AF and AFrelated morbidity is enormous.Atrial fibrillation has a multiplicity of causes ranging from genetic to degenerative, but hypertension and heart failure are the commonest and epidemiologically most prevalent conditions associated with AF as both have been shown to create an arrhythmogenic substrate. Several theories emerged regarding the mechanism of AF, which can be combined into two groups: the single focus hypothesis and the multiple sources hypothesis. Several lines of evidence point to the relevance of both hypotheses to the mechanism of AF, probably with a different degree of involvement depending on the variety of AF (paroxysmal or persistent). Sustained AF alters electrophysiological and structural properties of the atrial myocardium such that the atria become more susceptible to the initiation and maintenance of the arrhythmia, a process known as atrial remodeling. Angiotensin II has been recognized as a key element in atrial remodeling in association with AF opening the possibility of exploitation of "upstream" therapies to prevent or delay atrial remodeling. The clinical significance of AF lies predominantly in a 5-fold increased risk of stroke. The limitations of warfarin prompted the development of new antithrombotic drugs, which include anticoagulants, such as direct oral thrombin inhibitors (dabigatran) and factor Xa inhibitors (rivaroxaban, apixaban). Novel mechanical approaches for the prevention of cardioembolic stroke have recently been evaluated: percutaneous left atrial appendage occluders, minimally invasive surgical isolation of the left atrial appendage, and implantation of carotid filtering devices.

Non-valvular atrial fibrillation (NVAF) increases 5 fold the risk of ischemic stroke 3 and it is responsible for 15-30% of all ischemic strokes AbstrACtLeft atrial appendage occlusion (LAAO) appears as a therapeutic option for some atrial fibrillation patients not suitable for oral anticoagulation because an increased hemorrhagic risk or recurrent ischemic events despite anticoagulant treatment. Methods: Report of consecutive atrial fibrillation patients treated with LAAO with Amplatzer Cardio Plug because contraindication or failure of oral anticoagulation with acenocumarol. CHA2DS2VASC, HAS-BLED, NIHSS, mRS, procedural complications and outcome were assessed. Seven patients (73 ± 6 year-old) were treated after intracerebral (n = 5) and gastrointestinal (n = 1) hemorrhages or ischemic stroke recurrence while on acenocumarol (n = 1). Results: Mean follow up was 18 months. Baseline CHA2DS2Vasc y HAS-BLED scores were 5.6 ± 0.7 and 4.1 ± 0.3 respectively. There were no strokes or deaths. There was only one non-serious adverse event. Conclusion: LAAO with ACP appears as a feasible therapeutic option for stroke prevention in patients with atrial fibrillation and failure or contraindication to acenocumarol.Keywords: atrial fibrillation, stroke recurrence, intracerebral hemorrhage, oral anticoagulants, Amplatzer Cardio Plug. resumen La oclusión de la orejuela auricular izquierda (OOAI) es una opción terapéutica en pacientes con fibrilación auricular y alto riesgo hemorrágico o recurrencia de accidente cerebrovascular isquémico (ACVi) a pesar del tratamiento anticoagulante. Métodos: Reporte de pacientes con fibrilación auricular y contraindicación o fallo terapéutico con acenocumarol tratados con OOAI. Se evaluaron escalas CHA2DS2VASC, HAS-BLED, NIHSS y mRS, complicaciones procedurales y resultados. Resultados: Siete pacientes (73 ± 6 años) fueron tratados luego de sufrir hemorragia cerebral (n = 5), gastrointestinal (n = 1) o ACVi recurrente a pesar del tratamiento con acenocumarol. Las escalas CHADS2VASC y HAS-BLED fueron 5.6 ± 0.7 y 4.1 ± 0.3 respectivamente. Luego de un seguimeinto promedio fue de 18 meses (3-50) no se registraron ACVi o muertes. Se registró sólo un evento adverso no serio. Conclusión: La OOAI es una opción terapéutica factible para prevenir ACVi en pacientes con fibrilación auricular y fallo o contraindicación para recibir acenocumarol.Palabras-clave: fibrilación auricular, hemorragia intracerebral, anticoagulantes orales, Amplatzer Cardio Plug.
