Abstract: Objective To determine the prevalence of lupus anticoagulant and raised anticardiolipin antibodies Design A prospective study of lupus anticoagulant and anticardiolipin antibody levels in Setting The prepregnancy clinic and miscarriage antenatal clinic in a tertiary referral centre. Subjects Two hundred and forty-three women, of whom 113 (47%) had a past history of two miscarriages, and 130 (53 YO) had three or more miscarriages. “…Several series have been described that show an association between LA and RSA. 5,8 However, LA levels can vary during pregnancy, 7 and persistently elevated levels are strongly associated with gestational loss. 5 We did not find a good correlation between this test and RSA in this group of patients.…”

“…5 Simultaneous presence of LA and other APA seems to have an important association with early abortion. 8 Inherited factors for thrombophilia include protein C, protein S and antithrombin III (ATIII) deficiencies, factor V Leiden and G20210A mutations in the prothrombin gene and C677T mutation in the methylenetetrahydrofolate reductase (MTHFR) gene.…”

“…Grandone [70] assessed 32 patients with RMS and found 28% to have ACLAs, and Birdsall [34] studied 81 patients with RMS, finding 41% to harbor ACLAs. Maclean [107] assessed 243 patients with RMS ( two spontaneous abortions) and found 17% to have ACLAs, 7% to have LAs, and 2% to harbor both. Howard [83] assessed 29 nonlupus patients with RMS and found 48% to have LAs.…”

“…The neurologic syndromes associated with ACLAs include TIAs, small-stroke syndrome, arterial and venous retinal occlusive disease, cerebral arterial and venous thrombosis, migraine headaches, Degos' disease, Sneddon's syndrome [104], Guillain-Barré syndrome [106], chorea, seizures, and optic neuritis [101,107,108]. The CNS manifestations of systemic lupus erythematosus are commonly, but not always, associated with positive antiphospholipid antibodies [109,110].…”

“…These pathological changes in the placenta may result in miscarriage, IUGR, stillbirth, and early severe preeclampsia. In relation to fetal loss, positive test for aCL or presence of LAC may be found in up to 20% of women with recurrent pregnancy loss [31][32][33][34][35][36]. It may present with either recurrent embryonic loss [37], or fetal demise beyond 10 weeks of gestation [38] and is found in 10-15% of women with fetal death beyond 20 weeks of gestation [39,40].…”
