The predictive role of serum and bronchoalveolar lavage cytokines and adhesion molecules for acute respiratory distress syndrome development and outcome

Abstract: Background: The predictive role of many cytokines and adhesion molecules has not been studied systematically in acute respiratory distress syndrome (ARDS). Methods: We measured prospectively tumour necrosis factor alpha (TNF-α), interleukin (IL)-1, soluble vascular adhesion molecule-1 (VCAM-1) and soluble intercellular adhesion molecule-1 (ICAM-1) in serum and bronchoalveolar lavage fluid (BALF) within 2 hours following admission, in 65 patients. The patients were divided into: those fulfilling the criteria fo… Show more

“…NF-kB activation typically involves the phosphorylation of IkB by the IkB kinase (IKK) complex, which results in the degradation of IkB, and subsequent release of NF-kB, allowing it to translocate freely to the nucleus. Upon exposure to LPS, activated NF-kB can release numerous cytokines, including TNF-a, IL-6 and IL-1b, which play a critical role in the generation of ALI (Agouridakis et al, 2002;Bhatia & Moochhala, 2004).…”

“…[48][49][50][51] Interestingly, the development of ARDS can be predicted by the levels of sVCAM-1 in the serum, much like cardiovascular disease. 52 Therefore, it is likely that treatment with PTX will not only decrease the severity of cardiovascular risk in HIV patients but may also reduce the risk of pulmonary disorders.…”

“…Other soluble fragments of adhesion molecules, such as sP-and sE-selectin, or soluble intercellular adhesion molecule-1, in plasma and bronchoalveolar lavage fluid have been suggested as predictors for the onset and outcome of inflammatory pulmonary complications as well (42,48,53,54 ), and combining these tests may increase the discriminatory power of a prognostic model. However, collecting almost complete information by systematically exploring a broad range of data sources needs to narrow the molecule of interest.…”
