RESUMENCon la denominación enfermedad linfoproliferativa postrasplante (ELPT) se conoce un grupo de trastornos que se pueden presentar con posterioridad al trasplante de órganos sólidos, con incidencia variable dependiendo del tipo de órgano trasplantado. La presentación clínica inespecífica de esta enfermedad, el compromiso extranodal y su amplio espectro histopatológico hacen que tanto la clasificación como el tratamiento sean complejos. Esta revisión presenta una actualización sobre la ELPT en pacientes con trasplante de órgano sólido. PALABRAS CLAVE Infecciones por Virus de Epstein-Barr; Trasplante de Órganos; Trastornos Linfoproliferativos SUMMARY Post-transplant lymphoproliferative diseasePost-transplant lymphoproliferative disease (PTLD) is a group of disorders that may occur after transplantation. Its incidence is variable according to the transplanted organ. The clinical variability of this disease, its extra-nodal involvement and its broad histopathological spectrum make both classification and management very complex. This review provides an update about PTLD in patients with solid organ transplantation.

Ö ÖZ ZE ET T Posttransplant lenfoproliferatif hastalıklar (PTLH), solid organ transplantasyonu veya allojeneik hematopoietik kök hücre transplantasyonu (HKHT) sonrası immünsupresyona ikincil gelişen, benign poliklonal lenfoid proliferasyon ile malign monoklonal lenfoid proliferasyon arası spektruma sahip bir hastalık grubudur. Klinik tabloda belirgin morbidite ve mortalite riski mevcuttur, insidansı %1-30 arasında bildirilmektedir. Hastanın transplantasyon yaşı ve transplantasyon öncesinde Epstein-Barr virus (EBV) ile enfekte olma durumu, transplante edilen organın tipi, kullanılan immünsupresif rejim PTLH gelişmesini etkilemektedir. Dünya Sağ-lık Örgütü, PTLH için dört farklı morfolojik lezyon tanımlamıştır: Erken PTLH, polimorfik PTLH, monomorfik PTLH ve klasik Hodgkin lenfoma benzeri lezyonlar. Hasta değerlendirilirken alıcı ve vericinin EBV/sitomegalovirüs serolojisi; allogreftin tipi; transplantasyon sonrası ne kadar zaman geçtiği; alıcının transplantta yaşı dikkate alınmalıdır. PTLH immünolojik ve onkolojik bir hastalıktır. Günümüzde çocukluk çağı PTLH tedavisinde preemptif yaklaşımda uygulanan immünsupresyonun azaltılması, antiviral ajanların (± immünglobulin) yanı sıra standart tedavi yaklaşımı antrasiklin içermeyen, siklofosfamid, prednizolon ve rituksimab içeren azaltılmış doz kemoterapi uygulanmasıdır. Hedefe yönelik immün temele dayalı kombine tedavilerin kullanımı, rituksimab ve üçüncü parti EBV spesifik T-lenfositlerin kullanılmasıyla PTLH tedavisi güncel araştırma konularıdır. Günümüzde Türkiye'de giderek artan sayıda pediatrik solid organ transplantasyonu ve hematopoietik kök hücre transplantasyonu yapılmakta ve beraberinde giderek artan sayıda PTLH gelişen çocuk olgular bildirilmektedir. Ülkemizde çocuk hastalara özel bir transplantasyon kayıt sisteminin ve transplantasyon yapılan çocuk hasta grubunun gerek PTLH gelişme riski gerekse diğer onkolojik hastalıkların gelişme riski açısından standart bir izlem politikasının geliştirilmesine ihtiyaç bulunmaktadır. A 15-year-old Japanese female with end-stage kidney disease, kidney cysts, and angiomyolipoma due to tuberous sclerosis complex (TSC) received an ABO-matched preemptive kidney transplantation from her father. Basiliximab induction therapy was done on days 0 and 4, and tacrolimus, mycophenolate mofetil, and methylprednisolone were administered. Six months later, cervical lymphadenopathy developed, and computed tomography revealed an abdominal mass. Epstein-Barr virus-positive T cell post-transplant lymphoproliferative disorders (PTLD) was diagnosed. The pathology showed a monomorphic type lymphoma, and the Ki-67 index, a cell proliferation marker, was above 90%. The patient received four courses of CHOP (cyclophosphamide, doxorubicin, vincristine, and prednisolone) therapy, and tacrolimus was switched to everolimus, an inhibitor of the mammalian target of rapamycin (mTOR) pathway. Everolimus acts not only as an immunosuppressant, but also has an anti-tumor effect which may inhibit lymphoma development and proliferation. Three years later, the patient has shown no sign of PTLD recurrence. Her kidney function remains good, and a pathological examination detected no sign of rejection. In addition, her facial angiofibroma has improved. Although this study is based only on a single case observed over a short period of time, we consider everolimus to be a possible option in the treatment of PTLD after CHOP chemotherapy, especially in patients with TSC.
