Background The creation of a bioengineered cardiac patch (BCP) is a potential novel strategy for myocardial repair. Nevertheless, the ideal scaffold for BCP is unknown. Objective We investigated whether the decellularized placenta (DP) could serve as natural scaffold material to create a BCP for myocardial repair. Methods and results A BCP was created by seeding human-induced pluripotent stem cell-derived cardiomyocytes (hiPSC-CMs; 1 × 106/cm2) onto DP. The functional and electrophysiological properties of the BCP were first characterized by in vitro analysis and optical mapping. Next, in vivo therapeutic efficacy of the BCP was evaluated in a rat model of myocardial infarction (MI), created by left descending coronary artery ligation (MI + BCP group), and compared with MI alone (MI group), transplantation of DP (MI + DP group), and hiPSC-CMs (MI + CM group). Cytokine profiling demonstrated that the BCP contained multiple growth and angiogenic factors, including vascular endothelial growth factor, platelet-derived growth factor, insulin-like growth factor-1, basic fibroblast growth factor, angiogenin, and angiopoietin-2. In vitro optical mapping showed that the BCP exhibited organized mechanical contraction and synchronized electrical propagation. RNA sequencing showed that DP enhanced the maturation of hiPSC-CMs compared with the monolayer of cultured hiPSC-CMs. At 4 weeks follow-up, the BCP significantly improved left ventricular (LV) function, as determined by LV ejection fraction, fractional shortening, + dP/dtmax, and end-systolic pressure-volume relationship, compared with the MI, MI + DP, and MI + CM groups. Moreover, histological examination revealed that engraftment of the BCP at the infarct zone decreased infarct size and increased cell retention and neovascularization compared with the MI, MI + DP, and MI + CM groups. Conclusions Our results demonstrate that a DP scaffold contains multiple growth and angiogenic factors that enhance the maturation and survival of seeded hiPSC-CMs. Transplantation of a BCP is superior to DP or hiPSC-CMs alone in reducing infarct size and improving cell retention and neovascularization, thus providing a novel therapy for myocardial repair following MI.

Abstrak Penyakit jantung koroner secara klinis ditandai dengan nyeri dada akibat sumbatan di arteri coroner. Akupresur merupakan bagian terapi komplementer yang mampu meningkatkan kadar endorfin untuk merangsang penurunan nyeri. Pelaksanaan evidence based nursing akupresur ini diberikan pada 8 pasien dengan teknik pemilihan purposive sampling. Instrument penerapan menggunakan skala penilaian nyeri visual analog scale. Penerapan akupresur diberikan selama 20 menit pada titik akupresur L14 dengan skala nyeri 0 sampai 5. Hasil dari 8 sampel yang diberikan akupresur semua pasien mengalami penurunan skala nyeri. Penekanan atau sentuhan pada titik akupresur dapat meningkatkan kadar endorfin dalam darah maupun sistemik. Endorfin merupakan opiat tubuh secara alami dihasilkan oleh kelenjar pituitary yang berguna untuk mengurangi nyeri, mempengaruhi memori dan mood yang kemudian akan memberikan perasaan relaks. Terapi akupresur terbukti mampu menurunkan nyeri sehingga bermanfaat untuk diterapkan pada pasien akut koroner sindrom dengan keluhan nyeri dada. Abstract Coronary heart disease is clinically characterized by chest pain due to a blockage in the coronary arteries. Acupressure is part of complementary therapy that is able to increase endorphin levels to stimulate pain reduction. The implementation of evidence based nursing acupressure was given to 8 patients with a purposive sampling technique. The application instrument uses a visual analog pain scale rating scale. The application of acupressure was given for 20 minutes at the L14 acupressure point with a pain scale of 0 to 5. The results of the 8 samples given acupressure all patients experienced a decrease in pain scale. Emphasis or touch on the acupressure point can increase blood and systemic endorphin levels. Endorphins are the body's opiates naturally produced by the pituitary gland which are useful for reducing pain, affecting memory and mood which will then relax. Acupressure therapy has been proven to reduce pain so it is useful to apply to acute coronary syndrome patients with chest pain.

The human mesenchymal stem cells (hMSCs) therapy offering an encouraging the new methods to establish the conveying on the chitosan (C)/dextran (D)/β-glycerophosphate (β-GP) loaded with hMSCs to enhance the acute myocardial infarctions. The synthesized hMSCs-CD@β-GP system displayed the ratio of determination modules, size of the pore, absorbency, and the swellings ratio in the assortment of the 65 ka, 149 ± 39.8 µm, 92.2%, 42 ± 1.38, and 29 ± 1.9, respectively. The fabricated hMSCs-CD@β-GP was highly stable and physicochemical investigated and confirmed the suitability of the materials for cardiac regeneration applications. The in vitro examinations of the injectable hydrogels with hMSCs-CD@β-GP have recognized that the improved survival rate of the cells, increased the pro-inflammatory expressions factors, pro-angiogenic factors analysis confirmed the promising results of the ejection of fractions, fibrosis area, vessel density with decreased infractions size, with suggesting that the remarkable improvement of the heart regenerative function after myocardial infarctions. The new synergistic approach of the injectable hydrogels with hMSCs could able appropriate for the effective treatment of cardiac therapies after acute myocardial infarctions.
