The Pathologic and Genetic Characteristics of Extranodal NK/T-Cell Lymphoma

Abstract: Extranodal NK/T-cell lymphoma is a neoplasm of NK cells or cytotoxic T cells presenting in extranodal sites, most often in the nasal cavity. The typical immunophenotypes are cCD3+, sCD3−, CD4−, CD5−, CD8−, CD16−, and CD56+ with the expression of cytotoxic molecules. Tumor subsets express NK cell receptors, CD95/CD95L, CD30, MYC, and PDL1. Virtually all the tumor cells harbor the EBV genome, which plays a key role in lymphomagenesis as an epigenetic driver. EBV-encoded oncoproteins modulate the host-cell epigen… Show more

“…A common immunophenotype of NK-cell lineage ENKL is the presence of CD2, CD56, cytoplasmatic CD3 and cytotoxic markers (TIA-1, perforin and granzyme B) with the absence of surface CD3. Less frequently, the lymphoma cells originate from T cells and show a different phenotype often characterised by the presence of surface CD3, CD2, CD8 and/or CD7 1–5 9. ENKL has a strong association with EBV.…”

“…Epigenetic modifiers, including MLL2 , MLL3 , BCOR , TET2 , EP300 , and ARID1A , are mutated in about 24.8% of cases with ENKTCL ( 9 ). Accordingly, epigenetic regulation is an important treatment strategy for this disease.…”

“…Moreover, common genetic aberrations (18.2%) in ENKTCL involve genes related to the JAK-STAT pathway ( JAK3 , JAK2 , STAT3 , STAT5B , SOCS1 , and PTPRK ). JAK or STAT activating mutations lead to constitutive activation of the JAK-SATAT pathway, which plays a major role in ENKTCL cell growth and survival ( 9 , 201 ). These findings make JAK an interesting target for the treatment of ENKTC.…”

“…Interestingly, EBV may infect not only B-lymphocytes but even T-lymphocytes and NK-cells, although the mechanism for virus entrance into T/NK-cells is not well-known [ 34 , 35 , 36 , 37 , 38 ].…”
