A multitude of terms and definitions are used in patient safety related to medication. This heterogeneity makes it difficult to compare the results among studies and to appreciate the true magnitude of the problem. Classifying and unifying the terminology is necessary to advance in patient safety strategies.

“…Hospital admission, discharge, and follow-up programs have shown that when pharmacists are included in HIE and transitions of care efforts, enhanced problem detection and resolution occurs. 10 –19 However, similar efforts in community pharmacies are limited as community pharmacies’ inclusion in HIE is rare. Although the US Office of the National Coordinator envisions pharmacies’ involvement in HIE, current policy discourages community pharmacists’ engagement in HIE for 2 reasons.…”
In this pilot, community pharmacists identified more MRPs and omissions in preventive care when they reviewed unedited medical records. Larger studies are warranted to determine whether HIE can improve outcomes.
