The Organisation and Management of Hand Trauma Presenting to Plastic Surgical Services in the British Isles

Abstract: The workload of any trauma service varies considerably over short time periods. This is as true for hand trauma as it is for any other trauma specialty. however, the resources to manage the trauma caseload are relatively fixed. The aim of our study was to investigate the optimum way to manage hand trauma given its unpredictable workload. This information could be used to improve the efficiency of our local service and construct guidelines for other services to supplement the current guidance in circulation.

“…However, with this model, patients are triaged, given outpatient appointments, and may be waiting longer for surgery than the traditional model of care. Only 52% of plastic surgery hand trauma units in the United Kingdom have a dedicated weekend theater list, leading to patients with significant injuries potentially waiting days for surgical repair [5]. It, therefore, becomes paramount to adequately triage patients in accordance with best clinical evidence and predictors of poor clinical outcomes.…”

“…In our unit at the Royal Free Hospital we run at least one full-day trauma operating list every day, with between 5-15 day-case hand trauma patients per day. This equates to several thousand patients per year, a picture mirrored in the majority of UK plastic surgery units [3]. Despite high demand for services, there is limited operating capacity in most units, increasing pressure on the operating teams.…”
