ÖZET Amaç: Granülomatöz mastit, hem tanısı zor hem de ideal tedavi yöntemi olmayan kronik inflamatuar bir hastalıktır. Nadir görülen bir antite olması nedeni ile prognostik faktörler ve tedavi kalsifikasyonu üzerine araştırmalar kısıtlıdır. Son zamanlarda sistemik inflamasyonun biyobelirteci olarak kullanılan nötrofil-lenfosit oranı (NLO) nın popülerliği giderek artmaktadır. Bu çalışmada, kronik inflamatuar hastalık olması nedeni ile granülomatöz mastitte tedavi sonrası nükslerin öngörülmesinde tedavi öncesi NLO'nun rolünün araştırılması amaçlanmıştır. Gereç ve Yöntemler: Biyopsi sonuçlarına göre granülomatöz mastit hastalığı tespit edilen ve tedavi edilen 62 hastanın verileri retrospektif olarak incelendi. Hastalar, medikal tedavi ve cerrahi tedavi olmak üzere 2 gruba ayrıldı. Demografik verilerinin yanında tedavi öncesi NLO'ları hesaplandı. Her iki grupta da tam kür sağlanmış olan hastalar çalışmaya dâhil edildi. Tedavilerin tamamlanmasından en az 6 ay sonra hastalar telefon yoluyla aranarak nüks durumları, tekrar tedavi alıp almadıkları sorgulandı. Bulgular: Toplam 59 hasta çalışmaya dâhil edildi. Hastalar tedavi sonrası en az 6 ay takip edilmiş olup, ortalama takip süresi 52,44±27,29 (6-103) ay idi. Steroid tedavisi alan hastaların 4 (%20)'ünde nüks gelişirken, cerrahi tedavi alan hastaların 3 (%7,9)'ünde nüks gözlendi. Cerrahi ve medikal takip edilen hasta grupları arasında, tedavi öncesi NLO ile nüks açısından anlamlı farklılık izlenmedi (p>0,05). Ancak yapılan tedaviye bakılmaksızın incelendiğinde, nüks olan hastaların NLO'ları nüks olmayanlara göre daha yüksekti (p=0,00). Sonuç: Granülomatöz mastit hastalarında tedavi öncesi NLO değeri uzun dönemde nüksü öngörmede fikir verici olabilir. Total leukocyte count increases significantly in response to infection, trauma, inflammation, and certain diseases. Factors affecting leukocyte count in healthy adults include sex, hormonal milieu, genetic inheritance, stress level, diet, nutrition, and lifestyle (e.g. tobacco-induced inflammatory changes, chronic psychological stress, etc.). To date, numerous studies have reported that high but normal leukocyte counts at baseline predict increased cardiovascular and noncardiovascular mortality in older adults. Recent findings suggest that elevated leukocyte count within the normal range, but especially neutrophil and monocyte counts, may be a harbinger of increased systemic inflammation and subclinical disease. Moreover, elderly people who tend to have high but normal leukocyte counts are at greater risk of cancer, cardiovascular disease, type 2 diabetes, some other age-related conditions, and they also have increased all-cause mortality. These results indicate that strong and reliable inflammatory markers, such as leukocyte count, may reflect the rate of ageing and therefore can predict long-term survival in the elderly. Remarkably, leukocyte count correlates positively with genuine markers of systemic inflammation like C-reactive protein and interleukin 6. Interestingly, some authors conclude that leukocyte counts have a stronger prognostic ability with regard to total and cardiovascular mortality than total cholesterol or low-density lipoproteins. The fact that these inflammatory markers are clinically useful predictors of long-term survival in the elderly is quite remarkable as these blood parameters are included in routine medical check-ups. Therefore, they can be used as simple and reliable morphological indicators of chronic systemic inflammation, disease progression, and poor prognosis, especially among individuals who are likely to develop age-related conditions. Nevertheless, the pathomechanism that links elevated but normal leukocyte counts to increased mortality remains poorly understood. Introducere: Răspunsul inflamator joacă un rol critic în carcinogeneză. Există scoruri recente bazate pe răspunsul inflamator sistemic, cum ar fi raportul neutrofile/limfocite (NLR), raportul trombocite/limfocite (PLR), raportul limfocite/monocite (LMR), despre care s-a arătat că au valoare prognostică la pacienţii cu cancer. Aceste scoruri permit identificarea pacienţilor care vor avea un răspuns slab la tratament şi o supravieţuire de scurtă durată. Scopul acestui studio este de a evalua rolul prognostic al NLR, PLR, LMR şi PNI în termeni de supravieţuire la distanţă pentru bolnavii cu cancer colorectal, operaţi în urgenţă. Material şi Metode: Am inclus în studiu 391 de pacienţi internaţi şi operaţi pentru cancer colorectal complicat în clinica Chirurgie II a Spitalului Clinic Judeţean de Urgenţă "Sf. Ap. Andrei" din Galaţi, în perioada 2008-2017. Am analizat factorii paraclinici de inflamaţie sistemică NLR, PLR, LMR şi PNI.Ca factori de prognostic, au fost analizate curbele de supravieţuire. Rezultate: La pacienţii din lot, valorile crescute ale NLR, precum şi ale PLR au dus la creşterea riscului de deces (HR=7,581, 95% CI=(6.358,9.039), p value=0.000000, respective HR=1.043, 95% CI=(1.039, 1.047), p value=0.000000), iar valorile crescute ale LMR şi PNI au condus la scăderea acestui risc (HR=0.069, 95%
