The May–Husni Procedure: A Reappraisal

Abstract: These results indicate that a highly selected subgroup of patients with severe symptomatic post-thrombotic syndrome secondary to chronic segmental obstruction of the femoral vein do well after the May-Husni procedure. In order to refine the criteria for the selection of patients who may benefit from this operation, there is a need for more studies that use a combination of hemodynamic and validated scales that diagnose and grade the severity of post-thrombotic syndrome.

“…The only operation described in any detail is the May-Husni procedure, which is designed to relieve unilateral FV outflow obstruction by ipsilateral saphenous vein transposition to the distal FV or POPV. 387 In many cases, this may also include some form of endophlebectomy of the POPV. Obstruction is relieved by rerouting blood flow through the GSV, which subsequently drains the calf.…”

“…There is no relevant literature examining the efficacy of surgical thrombectomy compared with control groups in prospective randomized trials. Observational case series demonstrating symptomatic improvement after surgical endovenectomy with iliac vein stenting [38,39], and with saphenofemoral venous bypass [40], have been described.…”
