The Link Between Substance Abuse and Posttraumatic Stress Disorder in Women:A Research Review

Abstract: Research has documented a high incidence of comorbid post-traumatic stress disorder (PTSD) and substance abuse. Women substance abusers, in particular, show high rates of this dual diagnosis (30% to 59%), most commonly deriving from a history of repetitive childhood physical and/or sexual assault. Rates for men are two to three times lower and typically stem from combat or crime trauma. “…It is well established that the severity of childhood trauma is associated both with levels of substance use and the severity of dissociation (Diseth, 2005;Najavits & Walsh, 2012;Najavits, Weiss, & Shaw, 1997). Three times higher rates of substance abuse in subjects with vs. without a history of childhood trauma have been reported in the general population (Kilpatrick, Saunders, & Smith, 2003), and the severity of childhood trauma is closely linked to the level of substance abuse (Khoury, Tang, Bradley, Cubells, & Ressler, 2010;Scheidell et al, 2018).…”

“…This common comorbidity affects multiple domains of functioning and presents complex challenges to recovery (Najavits et al, 2008). The presence of either problem alone increases the risk of developing the other (Najavits, Weiss, & Shaw, 1997). Individuals with this comorbidity may enter high-risk environments when using substances, increasing their likelihood of further trauma.…”

“…The link between posttraumatic stress disorder (PTSD) and substance use in women is well established (Kessler, 1995; Lisa M. Najavits, Weiss, & Shaw, 1997). However, this association has not been well studied during pregnancy.…”

“…Seeking Safety focuses on eliminating substance use, reducing PTSD symptoms and increasing safety from HIV risk, domestic violence and self-harm (Brown et al, 2007; L. M. Najavits et al, 1996; Lisa M. Najavits, Gallop, & Weiss, 2006; Lisa M. Najavits et al, 1997). Seeking Safety has been tested primarily in women, but with mixed results (Brown et al, 2007; McHugo & Fallot, 2011; Lisa M. Najavits et al, 2006).…”
