A Multidisciplinary Approach to the Treatment of Co-occurring Opioid Use Disorder and Posttraumatic Stress Disorder in Pregnancy: A Case Report

Abstract: Perinatal opioid use disorders negatively impact maternal and neonatal outcomes and are a public health problem of increasing severity. More than half of women with a substance use disorder have a history of posttraumatic stress disorder that, if not adequately addressed, can impede substance use disorder treatment. “…Although much of the work identified in the review concerned PTSD’s effect on OAT, it is also important to understand the effects of OAT on PTSD symptoms. One case study described the integration of BUP into other medical care in the case of a pregnant woman with OUD and PTSD (Goodman, Milliken, Theiler, Nordstrom, & Akerman, 2015). In the case study, BUP was readily integrated and coordinated with other psychiatric care for PTSD (i.e., selective serotonin reuptake inhibitor), and pre- and postnatal care.…”

“…In the case study, BUP was readily integrated and coordinated with other psychiatric care for PTSD (i.e., selective serotonin reuptake inhibitor), and pre- and postnatal care. She reported improved PTSD symptoms and obtained more stable housing (Goodman et al, 2015). It may be that integration of services contributed to these favorable outcomes.…”

“…Lifetime and current trauma experiences add additional complexities to SUD treatment for postpartum women as trauma increases the risk of substance use recurrence and overdose (Cleveland et al, 2020). Evidence suggests that routine screening for trauma experiences and trauma-informed, integrated interventions that address both SUD and trauma history simultaneously improve treatment outcomes (Goodman et al, 2015;Marsden et al, 2019;Najavits et al, 1998); therefore, trauma experiences should be considered during SUD treatment planning for postpartum women.…”

“…Yet, a recent secondary database review from publicly available data sets by Terplan et al examined 13,000 addiction treatment facilities in the USA and found that trauma-related services were selectively available in urban centers and states with larger populations, and a large percentage of women in treatment programs reported unmet needs [ 39 •]. A case study by Goodman et al examined the complexities of providing obstetric care for women with comorbid PTSD and substance use and stressed the importance of early screening and a coordinated multidisciplinary approach to improving outcomes for the substance-exposed mother-infant dyad [ 40 ].…”
