The Lateral Transorbital Canthopexy for Correction and Prevention of Ectropion

Abstract: Background: There are numerous approaches to correcting laxity of the lateral canthal tendon, each with advantages and drawbacks. Critical evaluation of these techniques is not possible, however, as there is no grading system currently in use to describe this condition or to report outcomes, and prospective trials are lacking. Objectives: To report and assess a new procedure for repair of the lateral canthus (lateral transorbital canthopexy) and to describe the Ectropion Grading Scale (EGS), with a prospective … Show more

“…In 6 patients, lid loading had been performed for surgical rehabilitation of the lagophthalmus as a stand-alone procedure (Figs. 1 and 2), whereas in 16 patients, medial and/or lateral canthopexy (15,16) had been additionally performed to lid loading with platinum chains (Spiggle & Theis Medizintechnik GmbH, Overath, Germany).…”

“…In surgical rehabilitation of paralytic lagophthalmus, it is important also to consider after-effects of the palsy on the lower lid, which can even lead to an ectropium, and perform an elevation of the lower lid rim, if necessary, for example, by medial or lateral canthopexy (15,16). If the weight of the lid implant is only increased without elevation of the lower lid, the risk for complications of lid loading is needlessly increased.…”

“…2 Occasionally one must revert to fixating the LCT on the lateral wall itself. 3 Quite often after burring holes in the region of the frontozygomatic suture, it can be quite difficult to thread the suture through and grasp it on the other side efficiently. We have developed a technique whereby a 21-gauge needle is introduced into the hole, from outside in.…”

“…To demonstrate this, we modified the original Ectropion Grading Scale (EGS) described in 2000 1 to evaluate malposition of the medial and lateral lower eyelids individually ( Table 1). Use of the EGS thus assists in the first priority of treating lower eyelid malposition: correct characterization of the underlying malposition.…”
