The Laryngeal Mask Airway: A Method to Improve Visualisation of the Trachea and Larynx during Fibreoptic Assisted Percutaneous Tracheostomy

Abstract: A modification of the Ciaglia multiple-dilator method for percutaneous tracheostomy using both a fibreoptic bronchoscope and laryngeal mask airway to visualise the upper airway, so as to aid in safer placement, is described.

“…[58] In particular, the airway device is in a stable position and lies outside the larynx minimizing the risks of dislodgement, glottic damage and tube transfixion. [4] Fiber optic Bronchoscopic examination may be performed from the laryngeal inlet allowing accurate identification of the laryngeal rings and avoiding risk of damage to the bronchoscope.…”

“…The tip of the tracheal tube may still lie low enough within the trachea to risk cuff puncture or tube transfixion by the needle or by the guide wire. [4] Use of the classic laryngeal mask airway (LMA)[57] or the intubating LMA[8] during percutaneous dilatational tracheostomy (PDT) circumvents many of these problems. However, patients requiring PDT often have poorly compliant lungs requiring high inflation pressures.…”

“…Fernandez and his colleagues [29] suggested that because of the problems cited above, major complications could be avoided by continuous bronchoscopic observation during the procedure. The use of the laryngeal mask and bronchoscope may offer an alternative approach [31]; however, further controlled studies are required. “…Dexter 5 multiple-dilator method. In five patients the procedure was completed successfully using an LMA, though a large air leak was noticed in the first patient in whom the nasogastric tube was left in situ.…”
