Purpose:The main goal of this study is to discover the scientific evolution of Cancer-Related Symptoms in Complementary and Alternative Medicine research area, analyzing the articles indexed in the Web of Science database from 1980 to 2013.Design/Methodology/Approach: A co-word science mapping analysis is performed under a longitudinal framework (1980 to 2013). The documental corpus is divided into two subperiods, 1980-2008 and 2009-2013. Thus, the performance and impact rates, and conceptual evolution of the research field are shown.Findings: According to the results, the co-word analysis allows us to identify 12 main thematic areas in this emerging research field: anxiety, survivors and palliative care, meditation, treatment, symptoms and cancer types, postmenopause, cancer pain, low back pain, herbal medicine, children, depression and insomnia, inflammation mediators, and lymphedema. The different research lines are identified according to the main thematic areas, centered fundamentally on anxiety and suffering prevention. The scientific community can use this information to identify where the interest is focused and make decisions in different ways.Research limitation: Several limitations can be addressed: 1) some of the Complementary and Alternative Medicine therapies may not have been included; 2) only the documents indexed in Web of Science are analyzed; and 3) the thematic areas detected could change if another dataset was considered. Practical implications:The results obtained in the present study could be considered as an evidence-based framework in which future studies could be built.Originality/value: Currently, there are no studies that show the thematic evolution of this research area.

Introdução: A dor é um dos sintomas mais prevalentes em pacientes com câncer, especialmente no último ano de vida, sendo frequente o controle inadequado. Objetivo: Delinear o perfil de medicamentos utilizados para controle de dor em uma unidade hospitalar de cuidados paliativos oncológicos. Método: Estudo transversal com acompanhamento longitudinal de todos os pacientes internados entre setembro e novembro de 2016. Escala Verbal Numérica (EVN), funcionalidade e medicamentos com potencial analgésico utilizados foram coletados por revisão de prontuário. Resultados: Os 399 pacientes internados compuseram 461 episódios de internação, sendo 429 (93%) com o sintoma dor (controlada ou não). A idade média foi 62 anos, oito dias em média de internação, motivada por dor em 18% dos casos; e, em 35%, o sintoma dor não estava controlado. Destes, a EVN foi quantificada como zero após dois dias em média. Segundo a escada analgésica, 29% estavam no primeiro degrau, 11% no segundo e 82% no terceiro. O uso do analgésico comum e de adjuvante foi verificado em mais de 80% dos episódios. A dose média equivalente de morfina oral foi 117 mg/dia. Discussão: O controle de dor observado foi superior e mais precoce se comparado com outros trabalhos semelhantes. A dose média de opioide (equipotência analgésica) foi semelhante a observada em outros estudos. Entretanto, o uso de opioide forte e de adjuvantes foi mais frequente do que o notado em outros serviços. Essa diferença pode justificar o melhor controle álgico. Conclusão: A ação da equipe especializada proporciona melhor controle de sintomas.

Background Early identification of patients requiring palliative care is a major public health concern. A growing number of instruments exist to help professionals in identifying these patients, but thus far, none have been thoroughly assessed for criterion validity. In addition, no currently available instruments differentiate between patients in need of general vs. specialized palliative care, and most are primarily intended for physicians’ use. This study aims to develop a new interprofessional instrument allowing identification of patients in need of general vs specialized palliative care.Methods The instrument development consisted of four steps: i) literature review to determine the concept to measure; ii) generation of a set of items; iii) review of the initial set of items by experts to establish the content validity; iv) administration of the items to a sample of the target population to establish face validity. We conducted a Delphi process with experts in palliative care to accomplish step 3 and sent a questionnaire to nurses and physicians non-specialised in palliative care to achieve step 4. A committee of interdisciplinary clinical experts supervised all steps.Results The study was conducted in the French - and Italian-speaking regions of Switzerland. The literature review confirmed the necessity of distinguishing between general and specialised palliative care needs and of adapting clinical recommendations to these different needs. Thirty-six nurses and physicians participated in the Delphi process and 28 were involved in the face validity assessment. The Delphi process resulted in two item lists: a 7-items list to identify patients in need of general PC and an 8-items list for specialised PC needs. The content and face validity were deemed to be acceptable by both the expert and the target populations.Conclusion This instrument considers a new perspective in the identification of patients requiring palliative care as it has been designed to differentiate between general and specialised palliative care needs. Moreover, diagnostic data is not fundamental to the use of the instrument, thus facilitating its use by healthcare professionals other than physicians, in particular nurses. Internal and criterion validity assessments are ongoing and essential before further dissemination of the instrument.

Abstract: The methodological problems and publication bias call for higher-quality studies to be designed, funded, and published. However, there is a clear message that SPC is multi-disciplinary and aims at palliation of symptoms and burden in line with current recommendations.
