The impact of bowl size, program setup, and blood hematocrit on the performance of a discontinuous autotransfusion system

Abstract: BACKGROUND: Cell salvage is an essential element in the concept of blood management. Modern devices provide different bowl sizes and sensor-directed programs to optimally adjust to varying clinical situations. STUDY DESIGN AND METHODS:In an experimental performance study, the discontinuous autotransfusion device XTRA (LivaNova/Sorin) was evaluated using fresh donor blood anticoagulated with heparin 5 U/mL and adjusted to a hematocrit of 10% or 25%, representing orthopedic or cardiac surgery. Test blood was pro… Show more

“…Plasma from the patient is effectively removed during the processing of blood with CS. 21 Plasma contains platelets with direct antimicrobial activity, as they are activated to release peptides in response to trauma or mediators of inflammation, 22 for this reason pure platelet rich plasma is applied topically, for instance in oral surgery. 23 Plasma of cardiac surgical patients contains, as a result of the inflammatory response caused by surgery and CPB, pro-and anti-inflammatory enzymes and cytokines, such as interleukines, tumor necrosis factor, myeloperoxidase, elastase and complement factors.…”

“…Albumin is confirmed to best represent plasma elimination, while PFH and potassium show markedly lower values and wider variation. Calculation of their washout is obscured by the fact that their concentration decreases during the wash phase but concomitantly increases with some haemolysis occurring during the process . Heparin results overestimate plasma elimination due to additional removal by adsorption to cells and artificial surfaces .…”

“…Clinical validation of the Sorin Xtra has shown the consistency of its hematocrit sensors and PRC product. 12,13 Fresenius CATS *plus The CATS *plus device was the original cell salvage system from Fresenius that was developed to maximize blood collection for all sized patients ( Figure 2). The CATS *plus is a centrifugal system that utilizes a continuous process for an autotransfusion product.…”
