The hypoxic burden of sleep apnoea predicts cardiovascular disease-related mortality: the Osteoporotic Fractures in Men Study and the Sleep Heart Health Study

Abstract: Apnoea-hypopnoea index (AHI), the universal clinical metric of sleep apnoea severity, poorly predicts the adverse outcomes of sleep apnoea, potentially because the AHI, a frequency measure, does not adequately capture disease burden. Therefore, we sought to evaluate whether quantifying the severity of sleep apnoea by the 'hypoxic burden' would predict mortality among adults aged 40 and older.

“…Also, it has been suggested that PLMS may actually be evoked by basal sympathetic hyperactivity (Guggisberg, Hess, & Mathis, 2007 (Prabhakar, Kumar, & Peng, 2012). Notably, hypoxaemia is more closely related to adverse outcomes than AHI and other OSA markers (Azarbarzin et al, 2019;Lavie, 2015). (Gottlieb, Somers, Punjabi, & Winkelman, 2017;Guyon et al, 2014;Meerlo, Sgoifo, & Suchecki, 2008).…”

“…The desaturation area is a manifestation of both desaturation depth and duration, and quantifies the area between the baseline and the SpO 2 trace during a desaturation, and therefore quantifies an overall hypoxaemic burden associated with events.…”

“…Azarbarzin et al . later used a similar approach to define ‘hypoxic burden’ as the total desaturation area across an overnight recording, and applied this metric to two independent prospective follow‐up studies (Sleep Heart Health Study and The Outcomes of Sleep Disorders in Older Men study) where it was a sensitive predictor of cardiovascular mortality in elderly men (hazard ratio ≈1.6–2) …”

“…In a recent study, the area under the oxygen desaturation curve associated with a respiratory event compared to baseline has been used to measure ‘hypoxic burden’. This measure was applied in cohort studies with PSG and long‐term follow‐up for health outcomes, showing that hypoxic burden, but not AHI, is associated with cardiovascular disease mortality …”

“…This measure was applied in cohort studies with PSG and long-term follow-up for health outcomes, showing that hypoxic burden, but not AHI, is associated with cardiovascular disease mortality. 55 The severity of the apnea event (measured as the product of apnea duration and desaturation area) was associated with increased mortality rates for patients with severe OSA. 56 Weight loss reduced the number of short duration events but did not change the number of longer events leading to the suggestion that the impact of weight loss on OSA reduction may be less previously thought.…”
